Care Resources PACE

Care Resources  PACE For most people, there are little to no deductibles or co-pays. Participants who do not meet PACE eligibility guidelines can pay for services privately.

Care Resources is a Program of All-inclusive Care for the Elderly, or PACE®, that provides wrap-around supportive services for people 55 years and older to keep them healthy and living independently in their homes. We partner with participants and their loved ones to develop a comprehensive care plan that meets the physical, mental, emotional and social needs of each participant and keeps them out

of nursing homes. Through our day centers in Grand Rapids and Lowell, we offer:
• Primary health care
• Home health care
• Clinic
• Prescription medication
• Rehabilitation services
• Activities
• Nutrition services
• Social services
• Transportation

PACE uses Medicare and Medicaid funds to cover all medically necessary care and services. Care Resources PACE serves those who are 55 and older and are:

A fresh look for a growing service committed to helping participants get where they need to go safely and comfortably. 🚌

Our transportation fleet now features a new look with our Doorway to Senior Independence message. In addition to updating our buses and minivans, we’ve added transit vans in response to growth in transportation needs.

Having a variety of vehicle options allows us to tailor rides based on route size, wheelchair accommodations and medical conditions. The result is a transportation program that stays flexible, efficient and participant-centered as our services continue to grow.

We were honored to attend the ribbon cutting for MoTown Square, a 54-unit affordable housing community designed to help older adults live safely, independently and with dignity.

This project reflects what’s possible when organizations come together around a shared commitment to supporting older adults in our community. Since October, our team has been working alongside partners to help connect future residents with the services and support they may need before moving in.

Through partnerships with PACE organizations like ours, the Area Agency on Aging of Western Michigan and Elders' Helpers, residents can access supportive services that help them maintain independence.

We’re proud to have participants living in the building and grateful to be part of the collaborative effort behind this important community resource.

- Personal care and daily support: bathing, dressing, meal preparation.
- Household services: light housekeeping, grocery shopping, laundry.
- Medical and nursing care: wound care, chronic condition monitoring.
- Equipment and supplies: walkers, wheelchairs, oxygen.
- Medication: management and delivery.

Throwback to May 2024 when we were constructing our building in Lowell! Did you know this was one of the first facilities in Michigan designed from the ground up with PACE participants in mind? Located at 11730 Fulton St. E, it features a state-of-the-art rehabilitation/therapy gym, a full-service medical clinic, and several participant activity areas both indoors and outside.

We were honored to attend the ribbon cutting and open house for Flat River Outreach Ministries' Main Street Housing in downtown Lowell this week.

Not only were the buildings beautiful, but they also help fill a tremendous need for affordable and supportive housing in the area. Helping residents remain safe, supported and connected in the communities they call home is near and dear to our hearts. It was special to be there to witness the start of something truly special. Congratulations to everyone who was involved in bringing this project to fruition!
