Community Health Alliance - Sojourner Recovery - Transitional Living TLC

Community Health Alliance - Sojourner Recovery - Transitional Living TLC Our programs include:

Sojourner Recovery Services, an agency of Community Health Alliance, is a full spectrum substance use treatment center.

Community Health Alliance, through Sojourner Recovery Services and Transitional Living-TLC, utilizes individualized treatment programs for general mental health and overcoming the disease of addiction. We have 7 residential facilities spread over Butler, Warren, and Preble County as well as 2 outpatient centers and our medical clinic located in Hamilton Ohio. We specialize in substance use treatme

nt for adult men, adult women, and pregnant or post-partum mothers. When you are referred to Sojourner Recovery for services, you can expect to receive an assessment on your first day either by telehealth or face to face by a certified addiction counselor to determine the appropriate level of care. We offer withdrawal management, residential treatment, partial hospitalization, intensive outpatient, and non-intensive outpatient. Childcare is offered during group time at several of our facilities, and we accept mothers with children under 12 at 2 of our residential locations. Medication Assisted Treatment
Sojourner offers the opportunity to be assessed by a physician for medication assisted treatment. The medications relieve withdrawal symptoms, reduce cravings, and bring about a biochemical balance in the body. We also can treat minor physical health conditions and work closely with several local primary care providers. Perinatal
Sojourner operates a prevention program focused on women of child-bearing age. We utilize a holistic approach to provide services to drug dependent and high risk pregnant and post-partum women before, during, after, or in lieu of treatment in a variety of settings. We strive to help women achieve abstinence and maintain sobriety throughout the last trimester of their pregnancy, thereby delivering a “drug free baby” and improving the quality of life for both mother and infant. Recovery Housing
Sojourner operates several Recovery Houses in Hamilton, Ohio. We have 3 houses for single men, 1 house for single women, and 1 house for pregnant women with children under 2. We offer sober support, peer support, and a client driven length of stay for everyone who chooses to reside in one of our recovery residences. Discharge Recovery Planning
Sojourner’s discharge planning services start at the point of admission and are an integral part of our recovery programs. We strive to provide clients with the skills and tools necessary to successfully transition once treatment is completed. Focus is placed on developing a recovery action plan which includes, among other things, sober support, housing, medical needs, employment, parenting and education.

Sometimes the mind can be loud. Thoughts, feelings, and past experiences can feel overwhelming—but they don't define who you are.

Take a moment today to notice your thoughts without becoming them. Feel your emotions without letting them control you. Remember that your story is still being written, and every day is a new opportunity to choose your next chapter.

We had such a great time supporting Butler County United Way at this year’s 2026 Golf United Classic at Shaker Run Golf Club! ⛳️

We’re proud to stand alongside so many incredible organizations working toward a common cause — strengthening Butler County by supporting health, education, recovery, housing, and basic needs services for individuals and families in our community. That mission is deeply connected to the work we do every day at Community Health Alliance through healthcare, behavioral health, recovery services, outreach, and support programs that help people find stability, healing, and hope. 💙

Representing our team this year were Amy Crouch, Executive Director of Transitional Living (TLC), Rebecca Dickerscheid, Executive Director of Sojourner Recovery Services, Keri-Anne Boller, CEO of Community Health Ohio, and Elise Powers, Manager of Admissions and Outreach. Thank you to Butler County United Way for bringing the community together for such a meaningful and fun event!

Through our agencies, Community Health Alliance supports healthier, more stable lives through behavioral health and recovery services across our region. https://communityhealthalliance.com/

Today is National Mental Health Provider Appreciation Day, and we want to recognize the incredible people who show up every day to care for others with compassion, patience, and respect.

We're giving a special shout-out to the providers at our Transitional Living (TLC) where they offer trauma-informed care that supports the whole person — body, mind, and community. Whether someone is facing anxiety, depression, trauma, recovery challenges, or simply trying to find their footing again, our team walks beside them every step of the way.

What makes this work so meaningful isn’t just clinical care — it’s the conversations, encouragement, understanding, and human connection that help people feel seen and supported.

To every counselor, therapist, nurse, case manager, peer supporter, and team member at TLC and across CHA: thank you for the care you bring to our community each and every day. 💚

Sometimes even through great effort, addiction gets the best of us. It takes courage — and a little humility — to dust ourselves off and try again. And if the original road didn’t work, maybe it’s time for a different path. We’ll help you figure it out.
