What actually happens when hospice comes to your home? 🤍

Hospice care is about so much more than a scheduled visit. It’s a whole team of support working together to help patients remain comfortable and help families feel informed and supported.

Depending on each patient’s needs, the hospice team may include nurses, physicians, hospice aides, social workers, chaplains and other professionals who work together to:

🤍 Manage symptoms and comfort
🤍 Review medications and changes in care
🤍 Educate and support caregivers
🤍 Answer questions as needs change
🤍 Provide support between scheduled visits
