I graded every way I've watched people pick a Medicare plan.

Two got an F. One got an S. Most people are using the F.

C: Whatever your neighbor has. You trust them. That's why it feels safe. But they don't take your prescriptions or see your doctors, so their good plan is built for their life, not yours.

C: Medicare.gov plan finder. Strong on medication costs. Weak on doctor networks. And it resets every January, so it's a project you inherit every year.

B: Read your Annual Notice of Change. Every year, no exceptions. It tells you what changed on your plan. It can't tell you whether a different plan now fits you better.

F: The biggest grocery or OTC card. I've watched people pick the plan with the highest card, then find out their doctors aren't in network. The extra benefits cards won't get you very far if you can't see the doctors you want.

F: The 1-800 number on TV. The carriers have enormous ad budgets and they spend them. Call it and you reach a call center several states away that doesn't know your situation and won't be there in March.

S: Start your own list. Doctors. Prescriptions. Pharmacy. Without it, nothing else on this list works, including me.

A: A local agent, every year. Costs you nothing. Knows the Lexington market. Compares every option in one place, and is still around when the bill looks wrong, or you're not sure the dentist is covered.

The most important piece of mail you'll get this year is about to show up. Most people won't finish reading it.

It's called the Annual Notice of Change. Every Medicare Advantage plan sends one in September.

It lists everything different about your plan in 2027. Premiums. Copays. Drug Cost. Which benefits changed.

And it reads like a legal document.

If you're already one of my clients, you know the drill. Bring it in, or don't even open it, and I'll walk you through it.

- What the plan charges when you actually use it

Two people in Fayette County started the same weight loss shot the same month in 2024.

Same drug. Same dose. Same BMI on the day they started. Both on Medicare.

This September, one of them is still paying full price at the counter. The other one pays $50.

The difference isn't their doctor, their plan, or their weight. It's one form.

Medicare has never paid for a weight loss shot. As of July 1 it does, through a test program called the GLP-1 Bridge. The video covers who qualifies and who doesn't. This is the part that decides which of those two people you are.

The BMI rule is measured from the day you started, not today. People who've already lost weight assume they've aged out. They haven't. The doctor attests to the starting number.

The order matters. Prescription to the pharmacy first. Then the pharmacy sends your doctor the form. Doctor files first, it bounces back "patient not found," and most people quit right there.

The $50 doesn't touch your deductible or your yearly cap. It's its own line.

It runs through December 2027. After that, nobody knows.

The full rules are on Medicare.gov. Search "GLP-1 Bridge." Print the fact sheet and take it to the appointment. Most offices haven't seen it yet.

It's "can I keep my doctor at UK HealthCare?" Or "is Baptist Health in this network?"

The answer is different plan to plan. And it changes year to year.

Nobody we work with enrolls until their doctors and their hospital are verified by name.
