Our focus is providing skilled nursing, short-term rehab, memory care and long-term care services in a compassionate, comfortable environment. Business hours are 8:00AM-4:15PM Monday-Friday
Suggested visiting hours are 8:00AM-8:00PM every day.

We had an amazing time at our first annual Pine Crest & Wood Aven Employee Picnic! Thank you to everyone who helped make the day such a success—The Smith Center for the wonderful space, Dug Out Games for the fun dunk tank, the Merrill Fire Department for filling it up, Arby’s for the delicious catering, and Pump Up the Party for the bounce houses.
From great food and face painting to bubbles and yard games, it was a day full of laughter and connection. Most of all, a heartfelt THANK YOU to our incredible employees—your hard work and dedication are what made this celebration so special. We appreciate you! 💙
