Hi, I'm Dr. Gabe Salloum, a board-certified plastic surgeon here in Miami Beach.

One of the most common questions I get is: how long does a facelift actually take?

For about 95% of my patients, I perform the facelift under sedation, not general anesthesia. There's an anesthesiologist present, patients receive an IV, and they're breathing completely on their own. No breathing tube. This "twilight" approach adds a little time, but it's absolutely worth it for patient comfort and safety.

A deep plane face and neck lift on its own takes me about 5 hours. It's a meticulous surgery and I don't rush it.

When we add complementary procedures like upper eyelid surgery, fat grafting, or a brow lift (which many of my patients do), the total time can extend to 7 or even 7.5 hours.

These surgeries are major restorations. The time invested is what makes the results look natural and last. My patients recover well from the sedation-based approach, and the results speak for themselves.

One of the most common questions I get from facelift patients is: where exactly are the incisions placed?

The short answer is that I place them very strategically so they stay hidden within natural contours. Most incisions follow the hairline, wrap around the ear, and tuck into the natural crease behind it. That way, once healed, they blend in seamlessly with your anatomy.

The longer answer is that every patient is different. Depending on your facial structure and the areas we're addressing, I may adjust the placement to give you the most natural result possible while keeping incisions as discreet as possible.

My goal is always for people to see that you look refreshed and rested, not that you had surgery. Where I place my incisions is a big part of how I achieve that.

"Am I a good candidate for liposuction?" I get this question all the time, so let's break it down.

Liposuction removes stubborn pockets of fat that just won't respond to diet or exercise, no matter how disciplined you are. But it's a body contouring procedure, not a weight loss solution, and being the right candidate matters more than people realize.

Here's what I look for:

I want you at or near your goal weight, with isolated fat deposits in specific areas like the abdomen, flanks, thighs, or arms. Good skin elasticity in the treated area is key since liposuction removes fat, not excess skin. You should be in good overall health, a non-smoker (or willing to quit before surgery), and have realistic expectations about what contouring can and can't do.

For technique, I often use ultrasound-assisted liposuction (UAL), which breaks down fat cells with ultrasonic energy before removal. This gives me more precision and helps the skin retract more smoothly than traditional methods. I also offer Renuvion J-Plasma alongside liposuction for patients who want additional skin tightening.

Recovery is more manageable than people expect. Most patients return to normal activities within three to fourteen days, with strenuous exercise resuming around the four week mark. Your final contour reveals itself gradually as swelling resolves over three to six months, so patience is part of the process.

Every body is different, which is why candidacy is something I evaluate individually at consultation, based on your health, your goals, and how your body holds fat and skin.

Patients ask me all the time how many facelifts I've done and what makes my technique different. After 20 years of specializing in this surgery, my answer is simple: experience changes everything.

When I first started, I was performing procedures very similar to a deep plane facelift. Over time, as our understanding of facial anatomy has deepened, so has my approach. I now go further and more aggressively into the face than I did as a younger surgeon, because I know that's where the long-lasting results live.

For patients who need it, I combine that with a more aggressive neck surgery as part of the same procedure. Not everyone requires that level of intervention, but when they do, the combination of a true deep plane facelift and comprehensive neck work is what gives my patients those natural, enduring results they're looking for.

"Will this keloid just come back?" It's the first question I get from almost every patient with a keloid on their ear, and it's a fair one. Surgery alone has a recurrence rate as high as 60%, which is why cutting it out by itself has never been my answer.

My protocol pairs surgical removal with same day, in office superficial radiation therapy (SRT). The excision addresses the raised tissue you can see. The radiation, delivered right after surgery while the incision is still fresh, is what actually keeps it from growing back. I consider the radiation the most critical part of this treatment, not an optional add on.

With this combined approach, I've brought recurrence rates down to under 10%. That's the difference between a temporary fix and a real solution.

These photos show one of my patients before and shortly after ear keloid removal. Early healing looks different for everyone, and outcomes vary based on your skin and history, but this is the kind of result this protocol is designed to deliver.

A question I get all the time: where do you actually perform the surgery?

I hold privileges at all the major hospitals in the area, including Mount Sinai, Baptist, Aventura, and Memorial. But for facelifts and most of my facial plastic surgery cases, I work out of an outpatient surgery center right near my office and Mount Sinai Medical Center.

Here's my honest reasoning: hospital surgeries tend to cost significantly more, and for these specific procedures, there's no real clinical advantage to being in a hospital setting.

What matters far more is the team. I work with the same anesthesiologist, nurse, surgical tech, and assistant on every case. That consistency and trust within the team directly impacts your experience and your outcome.

One of the most common questions I get before facial plastic surgery is: "Will I be put completely under?"

For most of my facelifts, eyelid procedures, brow lifts, and fat grafting, I actually prefer IV sedation over general anesthesia. You're still breathing on your own, fully relaxed and comfortable, and you skip the nausea, headaches, and grogginess that come with general anesthesia. My patients consistently tell me they feel so much better waking up from IV sedation.

Here's something else worth knowing: if you don't combine eyelid surgery with your facelift, your recovery is shorter and when it comes to pain, most of my patients are surprised. Tylenol is typically enough to keep them comfortable after surgery. It's rarely the ordeal people brace themselves for.

One of the most common questions I get is: what is recovery really like after a facelift?

The honest answer? It depends on whether you also have eyelid surgery.

If you combine a facelift with upper and lower eyelid surgery, expect swelling and bruising for at least four to five days. Keeping your head elevated and icing consistently makes a big difference during that window. The bruising itself usually fades over the following two weeks.

Here's what surprises most patients: the pain is actually quite manageable. The majority of my patients switch to extra-strength Tylenol very early in their recovery. A small minority needs anything stronger than that.

Beyond that, you may notice some swelling along the jawline and minor bruising around the neck. Many patients also feel a sense of tightness in the neck as things settle, and that is completely normal. It just takes a little time to adjust.

Recovery is very individual, but for most of my patients, it is far more manageable than they expected.
