PACE Your LIFE PACE Your LIFE is the first Program of All-Inclusive Care that is currently serving southern Kent County and northern Sussex County.

The Program of All-Inclusive Care for the Elderly (PACE) gives participants the ability to stay in their home while receiving essential services and maintaining independence and physical and emotional well-being. PACE Your LIFE provides comprehensive medical and social services to adults 55 and older that reside in the community, requiring assistance and mostly to individuals who are eligible for

Medicare and/or Medicaid benefits. A team of healthcare professionals provide PACE participants with personalized medical care.

What a great time at PACE Your LIFE’s First Responder Day! 🚒💙

Our participants and community members had a wonderful afternoon connecting with local first responders and learning more about the heroes who help keep our community safe every day.

A special thank you to Carlisle Fire Company for joining us and leading hands-on demonstrations, including fire safety tips, showing off firefighter gear, and sharing stories from their experiences serving the community. It was such a fun and educational event for everyone involved!

PACE offers many events like this throughout the year, along with fun activities, social opportunities, and personalized support that help seniors stay active, engaged, and connected.

Have you ever had the simple joy of painting a birdhouse? We did on campus yesterday!
​Connecting with our hands, making art, and staying creative is something that not only sticks with us as a core memory, but it's something that we can continue to make new memories doing!
​At PACE Your LIFE, located right here on the Milford Wellness Village campus, seniors are helped to stay independent, healthy, and creative while still living in the comfort of their own homes.
Through the all-inclusive care at PACE, everything is handled from comprehensive medical services to physical therapy, social activities and even art projects like this that keep sparking those core memories and creative vibes on and off campus.
​PACE serves seniors aged 55 and older across southern Kent and northern Sussex counties. The program is available to residents in the following Delaware zip codes: 19933, 19934, 19941, 19943, 19946, 19950, 19952, 19954, 19958, 19960, 19962, 19963, 19968, and 19979.
