Breast implants require periodic maintenance. Saline implants last for 20 years or more. Silicone implants only 10 to 12 years. Most patients prefer silicone, however. Those suitable for saline also do not need MRIs in the future. This patient decided to conveniently combine implant replacement with her “tummy tuck” procedure. At the same time there was an opportunity to fine tune the size she now preferred.

For the surgeons: The Hirmand classification of tear trough deformities does not include what could be termed a Type 0 which describes a pseudo-tear trough. The protruding fat simulates a tear trough but when the excess is removed there is no tear trough deformity revealed. That’s why in this case fat transposition was not necessary.

Skin was removed by raising a flap over the muscle. While the same amount of skin is removed as with pinch excision, detaching the established rhytids allows for smoother re-draping. It is tedious and takes longer but gives better results than the “lazy man’s” approach (lol).

When the number begins with an “8” surgery must be contemplated with safety uppermost in mind. A complete facelift is often unreasonable in this scenario because of the surgical time required. In this case a much more limited procedure consisting of a necklift and chin implant succeeded in producing a transformational result.

Plastic surgeons agree that male facelifts are much more challenging. Their larger scale adds operating time as does their greater tendency to ooze underneath the skin, which commonly bogs down the flow of the procedure. While skin quality as a fabric is better in men because it is more stout, age related loss of underlying fat can result in harsher contours, just as in women. Our patient, here in his late 60s, has achieved a rejuvenated look that is both subtle and natural. He is pleased and so are we!
