Preservation Rhinoplasty and Structural Rhinoplasty approach the nasal bridge in very different ways.
With Preservation Rhinoplasty, I can lower or reshape the bridge by removing small amounts of cartilage and bone from underneath it.
This allows the natural dorsum to remain intact rather than removing it, separating its components, and reconstructing the bridge.
That does not mean Preservation Rhinoplasty is appropriate for every patient. Certain anatomical problems still require a Structural approach.
When I can appropriately preserve the natural dorsum, I prefer to do so. The benefit for the patient is that we are maintaining the dorsum they were born with while changing its position and projection rather than rebuilding it.
This can create a more natural dorsal contour and helps avoid some of the irregularities that can occur when the dorsum is removed and then reconstructed.
