Wednesday, June 17th, 2026 12 PM - 3 PM
Clinic opens with appointment scheduling starting at 11:30 AM.
Physicians are available beginning at 1:00 PM.
Thursday, June 18th, 2026 4 PM - 7 PM
Clinic opens with scheduling starting at 3:30 PM.
Physician and PA availability and start times may vary.
Nurse health education and resource consults are available.
Walk-ins are welcome but limited.
If you choose to walk in, please arrive right at the start of clinic hours and be prepared for a wait as we work to accommodate you.
We strongly encourage you to call and schedule your appointment in advance to ensure the best possible care.
What to bring for a visit (new patients)
Photo ID
Verification of current address (bill or statement mailed to your current address)
Proof of household income (e.g., first page of federal tax return)

📢 Important Notice for Produce Perks Midwest Shoppers

We have learned that the Produce Perks program has been defunded and will no longer be available after July 1. Any remaining green Produce Perks tokens must be redeemed by June 26.

🌱 The good news: SNAP, EBT, and WIC shoppers can still take advantage of the Canal Market District Match Program thanks to the generous support of the Licking County Foundation.

The market will match up to $100 on SNAP/EBT/WIC transactions. Spend $100 in benefits and receive an additional $100 to shop the market.

These matching funds can be used on eligible food items throughout the market.

Again, if you have green Produce Perks tokens, please plan to use them by the Friday Farmers Market on June 26, 2026.
