Three diagnoses living in one square inch.

A marathoner in her early forties with heel pain, worst on the first steps out of bed. Be honest: you have already decided, haven't you?

But the fascia, the fat pad and the calcaneus share the same small space and the same training loads. Plantar fasciitis is common. Common still has to earn its place.

Here is the pivotal question: worse on the first steps, or worse the longer she stands?

If you're new to needling, this is where many start learning.

Let me introduce you to a muscle I love teaching: the infraspinatus.

It sits flat on the back of the shoulder blade, drives external rotation, and helps keep the cuff stable.

What makes it worth your attention is the referral. It can send pain deep into the front of the shoulder, the kind patients swear is inside the joint. Throwers and desk workers both show up with it.

Here is the concept I want new practitioners to hold: bone sits behind this muscle. That is part of why it is a common place to begin, and it is still a muscle you learn under supervision first.

I teach it this way at SmartCARE Education™. Share this with someone treating a shoulder that "feels like the joint."

You can't learn this from a video.

This is the room you want to be in. Not a lecture hall. A table, a patient, and an instructor close enough to see your hands.

The room, then the hands, then the lift. Prone. The upper trap lifted up off the rib cage. Nothing about it is rushed.

You can read about this muscle. You can watch videos of it. But the confidence comes from doing it with someone beside you who has done it many times and will tell you, quietly, when to adjust.

At SmartCARE Education™, that is the point. Not a list of needle locations. A system, learned up close, so the right tissue gets the right treatment for the right reason.

He walked in with a folder. Never a great sign.

Three clinicians in five months. Rest that seemed to work, until it didn't. A young volleyball player who had done everything he was told and was still stuck.

This is one of my favorite cases from The Clinical Coach™ by SmartCARE Education™, because the mistake in it is so common and so reasonable. Rest made the pain go away. Nobody asked what it did to his capacity.

The fix was not a new modality. It was reading the tendon correctly and loading it on purpose.

We all have a folder patient. Who is yours this week?

Most clinicians decide to needle before they've decided anything else.

Here is what I mean. Most courses teach where to put a needle. At SmartCARE Education™, I teach when you have earned the right to.

That is the Needling Gate. Four checks, in order. Fail one, and the gate closes.

Risk: contraindications, and the danger map for this region. On the upper trap, that map includes the lung.
Readiness: is this patient ready today, not last week?
Clarity: do you know which tissue you are treating, and why?
Needle: only now.

It runs on every patient, every visit. Every decision that matters happens before the needle comes out of the package.

Try the gate on your next patient, needle or not.

The rest worked. That was the problem.

Here is a story I see over and over. A tendon hurts, the athlete rests, the pain settles, and everyone calls it a win. Then they go back to the same training and it flares again within days.

Pain relief from rest is not capacity.

Rest calmed the tendon. It did not make it stronger. While he rested, his capacity dropped, so he returned to a wider gap between what the tendon could handle and what his sport asked of it than the one that hurt him in the first place.

The question is never just "does it hurt less?" It is "can it handle more?"

This one comes from The Clinical Coach™ by SmartCARE Education™. Share it with the colleague whose patients keep "resting it off."

Needle one of the most commonly needled muscles wrong, and you could puncture a lung.

I want to tell you why I teach this muscle the same way, every single time.

The upper trap is usually the first muscle a new practitioner wants to needle. Desk posture, stress, overhead athletes: those patients walk in every day. So does the lung, sitting closer than most people expect.

So at SmartCARE Education™, the upper trap never changes:
Prone, every time.
Lift the muscle up off the rib cage in a pincer grip.
A needle direction that never points toward the lung.

On a muscle this common, technique is the safety. If you are not sure you could explain all three of those steps, that is exactly where training starts.

Share this with the colleague who just started needling.
