When Caregivers Need Care: The Essential Role of Respite Support

Family caregivers throughout Maricopa County provide the majority of care for seriously ill loved ones. They manage medications, provide personal care,
handle household responsibilities, often while working full-time. Without breaks, even devoted caregivers burn out.

American Premier's certified nursing assistants provide respite care as part of home health and hospice services throughout Phoenix, Mesa, Scottsdale, and Glendale.
Our CNAs stay with patients so family caregivers can rest, run errands, attend their own medical appointments, work, or simply breathe.

During respite visits, CNAs provide all personal care: bathing, dressing, toileting, meals, companionship. They're trained professionals ensuring
patients are safe while family caregivers recharge.

Respite isn't luxury. It's necessity. Burned-out caregivers make medication errors, miss warning signs, develop their own health crises. Regular breaks help
caregivers sustain care over weeks or months.

Many feel guilty taking breaks. They shouldn't. Accepting help isn't abandoning responsibility. It's ensuring they can provide good care long-term.
Respite care is included in American Premier's services throughout Maricopa County.

The Medication Maze: Why Skilled Nursing Matters After Hospital Discharge

Hospital discharge often means multiple medication changes: new prescriptions, discontinued medications, adjusted dosages, complex instructions.
Medication errors are among the leading causes of hospital readmissions within 30 days.

American Premier's skilled nurses provide medication reconciliation during first home health visits throughout Maricopa County. We review every medication, identify what's new, what stopped, what changed throughout Phoenix, Mesa, Scottsdale, and Glendale.

We check for dangerous interactions. We organize medication schedules families can follow. We teach which side effects require immediate physician contact.
We identify medications taken incorrectly due to confusing instructions.

Medication management isn't just counting pills. It's preventing confusion that leads to missed doses, doubled doses, dangerous interactions, and ER visits. It's ensuring patients understand their treatment and can follow it safely. This is skilled nursing preventing complications throughout Maricopa County.

Older Americans Month: Celebrating Independence and Aging at Home

May is Older Americans Month, celebrating older adults and supporting their independence throughout Maricopa County.

American Premier helps older adults remain at home safely through home health services addressing challenges aging brings: managing chronic conditions, recovering from surgery, preventing falls, maintaining mobility, navigating healthcare needs.
Our physical therapists work on strength, balance, fall prevention.

Occupational therapists ensure patients can dress, bathe, perform daily activities independently. Skilled nurses manage medications and chronic conditions. Medical social workers connect families with community resources throughout Phoenix, Mesa, Scottsdale, and Glendale.

When independence at home is no longer possible, our hospice and palliative care ensure older adults receive dignified, compassionate care. This Older Americans Month, we celebrate the seniors we serve and honor their desire to age with dignity, independence, and choice.

"We focus on care coordination and teamwork. We meet weekly with each service line to discuss patients, innovate together."

Scott Scherpenberg, shares how our clinical-driven team approach ensures
seamless care throughout Maricopa County.

"At American Premier, we meet weekly with each service line to discuss patients, innovate together, discuss plans of care, how we can enhance things.
We focus on community resources and working as that one unit."

This matters especially when patients transition between service levels throughout Phoenix, Mesa, Scottsdale, and Glendale. "Whether that be a patient who transitions from palliative care to home health and then ends up transitioning to hospice or just somebody on an individual service line."

This is what sets American Premier apart: integrated care across home health, palliative care, and hospice under one roof. Weekly collaboration ensuring every
clinician knows every patient. Seamless transitions when care needs change.

The Unsung Heroes: Certified Nursing Assistants

While nurses provide clinical care and therapists work on rehabilitation, certified nursing assistants throughout Maricopa County provide personal care
that enables patients to maintain dignity at home.

American Premier's CNAs help with bathing, dressing, grooming, toileting, safe transfers, light housekeeping, and meal preparation throughout Phoenix, Mesa, Scottsdale, and Glendale.

Many patients can no longer bathe independently. Family members struggle with physical demands and emotional complexity of providing intimate care. CNAs bring skill and appropriate professional boundaries.

They recognize skin changes. They notice decreased appetite or confusion. They report concerns to nurses. They provide respite for exhausted family caregivers.

Personal care services are included in American Premier's home health and hospice throughout Maricopa County.

To our CNAs, thank you for the compassionate care you provide every day.

When Every Hour Matters: Same-Day Hospice Admission

Families throughout Maricopa County often wait too long to call hospice. When they finally realize hospice is needed, they need it now, not next week. American Premier offers same-day hospice admission throughout Phoenix, Mesa, Scottsdale, and Glendale because when symptoms are uncontrolled, when pain is overwhelming, when a family is exhausted, waiting isn't an option.

What same-day admission means:
You call in the morning, we're there that afternoon. Our nurse conducts assessment, orders medications and equipment, arranges delivery, begins comfort care immediately.

Within hours, patients have skilled nursing, pain management medications, medical equipment, 24/7 support, and coordinated care plan.

Why this matters:
Every day of uncontrolled symptoms is unnecessary suffering. Every day families struggle alone is a day without support. Every hour matters when time is limited.
Same-day admission is compassionate response to urgent need. It's understanding that hospice decisions are often made during crisis when families can't wait.

If your loved one needs hospice care throughout Maricopa County, don't wait. Call American Premier. We can be there today.

Mental Health Awareness Month: Supporting Caregivers and Patients

May is Mental Health Awareness Month. At American Premier throughout Maricopa County, we recognize that serious illness affects mental and emotional health as much as physical health.

Patients facing chronic illness or end-of-life often experience anxiety, depression, grief, and fear. Family caregivers shoulder immense emotional burden while managing medical care.

This is why medical social workers are essential to our teams throughout Phoenix, Mesa, Scottsdale, and Glendale. They provide emotional support, connect families with counseling resources, assess caregiver stress, and recognize when additional mental health support is needed.

Our bereavement coordinator offers grief counseling for 13 months. Our chaplains provide spiritual support. Our entire team understands that addressing mental health is part of comprehensive care.

This Mental Health Awareness Month, we remind families that asking for emotional support isn't weakness. It's essential to healing.
