Today is National Check Your Meds Day, a great reminder to take a closer look at the medications you or a loved one are taking.

If you or someone you care for takes multiple prescriptions, over-the-counter medications, or supplements, consider gathering everything and bringing it to a pharmacist for a review. They can check for interactions, flag anything outdated, and answer questions about dosage or side effects, often at no cost.

A few things worth asking:
✅ Are any of these medications no longer needed?
✅ Could any of these interact with each other?
✅ Is there a simpler or more affordable option?

For families managing care for an aging loved one, this is also something our Case Management Services can help with. Kristin Galle, RN, works alongside families to review medications and flag questions worth raising with a doctor or pharmacist.

What's the difference between our Home Care and Case Management services? And how do they work together?

What is Home Care?
Home Care provides hands-on, day-to-day support, things like companionship, meal prep, mobility assistance, and help with daily routines, all in the client's home.

What is Case Management?
Case Management works at a higher level, coordinating care across doctors and providers, supporting hospital and rehab discharges, reviewing medications, and helping families understand a new diagnosis or changing needs.

Do I need both?
Not necessarily. Some families only need Home Care. Others benefit from Case Management alone, especially when navigating a complex diagnosis or a lot of moving parts. Many families find the two work best together, hands-on care paired with someone coordinating the bigger picture.

How do they work together?
When both are in place, our Home Care team handles the daily details while Case Management makes sure nothing falls through the cracks between appointments, medications, and providers. It's one company, working from the same information, toward the same goal.
