At our 59-bed, Medicare and Medi-Cal certified skilled nursing facility, we pride ourselves on delivering high-quality, professional care in a warm, intimate setting. Our goal is for each resident to have optimal quality of life during his or her stay with us. To this end we provide physical, occupational, and speech therapies, as well as I.V therapy and enteral therapy. The restorative nursing pr

ogram enables residents to maintain the therapy gains they have achieved. As more and more residents, many in their 80's and 90's, enter the care center with Alzheimer's and Dementia, it's harder for them to reconnect to their memories in a lucid way. But thanks to a program called Music and Memory, in which Country Oaks Care Center is a participant, the residents there have a powerful tool to help them do just that.

Dear Family and Friends,
Fighting cancer takes immense strength, and no one should have to walk that path alone. This year, Valley Oaks Post Acute is proud to participate in the annual Day of Hope to raise vital funds for local cancer patients and their families.
On Wednesday, August 26, our team will be out in the community participating in the traditional Day of Hope special-edition newspaper sale! Every newspaper sold helps provide critical resources, treatment support, and hope to those fighting cancer.
