This is a drawing of the NMDA receptor that a patient made for me—and it happens to represent an important part of how ketamine works in the brain.

NMDA receptors are one type of receptor involved in the brain’s glutamate system, which plays an important role in communication between neurons.

Ketamine temporarily blocks NMDA receptors, setting off changes in glutamate signaling that are thought to promote neuroplasticity—the brain’s ability to form and strengthen new connections.

Ketamine produces rapid antidepressant effects, but its influence on glutamate signaling and neuroplasticity appears to be an important part of the story.

For some patients, depression isn’t temporary – it’s years of trying everything and still feeling stuck. This patient had been battling depression for eight years and had tried multiple treatments without relief. During a course of ketamine infusions, this patient’s PHQ-9 scores dropped from severe to minimal symptoms.
