When shopping carts were introduced in 1937, inventor Sylvan Goldman expected customers to love them.

They didn’t.

Men thought pushing a cart through a store was unmanly. Women disliked how closely the contraptions resembled baby carriages. Despite Goldman’s confidence in the idea, shoppers walked past the carts and carried their baskets by hand, the same way they always had.

The invention had come from a simple observation. Goldman had noticed that shoppers in his Oklahoma grocery stores tended to stop buying once their hand baskets became too heavy to comfortably carry. The solution, he figured, was to take the weight off their arms entirely. He developed a folding frame on wheels designed to hold two baskets at once, letting customers move through the store longer and buy more without the effort catching up with them.

The idea was sound. The public’s reaction was not what he had hoped.

So Goldman did something about it. He hired models, men and women both, to push the carts around his stores and demonstrate just how convenient they were. Seeing other people use them without embarrassment was enough to start shifting opinions. Slowly, then steadily, customers began reaching for the carts on their own.

What followed was one of the quiet revolutions of everyday life. The shopping cart spread from Goldman’s stores across the entire supermarket industry, eventually becoming so fundamental to the experience of grocery shopping that it is now impossible to imagine the layout of a modern store without it.

Sylvan Goldman became a multimillionaire. He had solved a problem most shoppers didn’t know they had, and built an industry around the answer.

Increase Cranial Pressure?

Cushing’s triad is a late warning sign that a traumatic brain injury (TBI) has progressed to imminent brainstem herniation. Waiting for these terminal signs to appear before intervening in a Prolonged Field Care (PFC / PCC) scenario means you are already behind the curve.

When an intracranial bleed causes pressure to build inside the rigid walls of the skull, the body triggers three classic physiological responses to maintain cerebral perfusion:

• Widening Pulse Pressure
• Bradycardia
• Irregular Respirations

The Strategy: Treat Early and Aggressively

Evidence from the CRASH-3 trial and Joint Trauma System (JTS) Guidelines proves that your window of opportunity sits entirely in the mild-to-moderate TBI stage. If you wait for Cushing’s triad to manifest, the survival benefit of your interventions drops exponentially because structural failure is already underway.

To stay ahead of intracranial pressure (ICP), PFC protocols dictate immediate, proactive management:

• Aggressive Clot Stabilization: Administer Tranexamic Acid (TXA) as a single 2g slow IV/IO push within the strict 3-hour window from the initial injury. Head trauma triggers hyperfibrinolysis, causing the body to prematurely melt its own clots. As an antifibrinolytic, TXA stabilizes those early clots to halt hematoma expansion.

• Proactive Physiology Management: Optimize cerebral venous drainage immediately by elevating the casualty’s head to 30 degrees and keeping the neck in a neutral alignment. Monitor aggressively to prevent the “deadly dual insults” of secondary brain injury: hypoxemia and hypotension.

• Osmotic Intervention: If signs of herniation or pupillary changes develop during prolonged holding, immediately transition to osmotic therapy by administering 3% Hypertonic Saline to reduce brain swelling.

When evacuation timelines stretch into hours or days, passive observation allows intracranial pressure to take over. Treat the underlying bleed early, control the physiology aggressively, and protect the brain before irreversible damage sets in.

Looking for some Spokane Inspiration??

Released in 1985, Vision Quest follows high school wrestler Louden Swain, played by Matthew Modine, as he pushes himself to drop from 190 pounds to 168 for a shot at undefeated three-time state champion Brian Shute.

Set in Spokane, Washington, the film mixes wrestling, extreme weight cutting, coming-of-age pressure, and romance into one of the sport’s most recognizable movies.

The film became a cult favorite in the wrestling community thanks to its intense training scenes, including the famous pegboard sequence, and its memorable soundtrack featuring Journey, Foreigner, and Madonna. Madonna also appears in the movie performing “Crazy for You” and “Gambler.”
