Isabella was born with Trisomy 18, also known as Edwards syndrome.

Her mom Ana is her primary caregiver, but balancing being a mom of 2 and 2 jobs? It took its toll.
That's when Ana learned about our respite services.

"That year was filled with uncertainty, heavy emotions and the constant fear that we might lose our daughter," says Ana. "During that time, my biggest concern about respite services was trust."
She wondered how someone coming into their home would adapt to their family dynamic, especially with a pr***en in the house and all the emotions the family carried.

The first time Ana saw how naturally the nurses cared for Isabella and how respectfully they treated her home, she felt something she hadn't felt in a long time: relief. The nurses were attentive, gentle, and aware of Ana's family as a whole.

"That was the moment I realized the respite care services were the right fit for us," says Ana.

What is "care coordination"?

We sat down with our Executive Director of Clinical Support Services Regina Stover to get an explanation of what care coordination is, how it works and why it's a critical part of the care Maxim provides.

Stover states that care coordination involves a wide range of people:
- aides
- therapists
- nurses
- social workers
- physicians
- payer case managers
- durable medical equipment companies
- pharmacies
- patients
- families
- authorized representatives
- recruiters
- discharge planners
- transitional care coordinators

Together, they form a network working toward a shared goal: safe, effective and seamless care.

When done well, it strengthens outcomes, reduces avoidable hospital visits and creates a more connected experience for patients and families.
