Transitions Healthcare Washington(formerly known as Humbert Lane) is a Skilled Nursing and Rehabilitation Facility that has served the community for over 29 years. We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

Food is more than what’s on the menu. For individuals managing diabetes and other chronic conditions, nutrition can be an important part of the overall care plan. 🩺

Our dietary and clinical teams work together to help residents and patients better understand choices related to carbohydrates, added sugars, portion sizes and balanced nutrition.

At Transitions Healthcare, education is part of empowering the individuals we serve to make informed choices, build healthier habits and take an active role in their health and well-being. 💙🍎

🌧️🎣 When the weather changes your plans, you make your own fun! 🎣🌧️

Our residents at Transitions Healthcare Washington PA were ready for some outdoor fun with water balloons! 💦🎈 Unfortunately, Mother Nature had other plans, so we brought the fun indoors and went fishing instead! 🐟🎣

Our residents had a wonderful time casting their lines, sharing plenty of laughs, and seeing what they could catch! 😄💙 Sometimes a rainy day leads to an even better adventure!

🎒✏️ Help Us Send Local Students Back to School! 📚🖍️

Transitions Healthcare Washington PA is hosting a Back-to-School Donation Drive to support elementary school students right here in our Washington community! 💙🏫

We’re collecting new backpacks and school supplies through Thursday, August 13. Donations can be dropped off at our front desk with the receptionist. From pencils and crayons to notebooks, folders and backpacks, every donation can help a local student start the school year prepared and confident! All supplies collected will benefit local Washington, PA elementary schools. ✏️📓🖍️🎒
