HiYa Home Care Co is proud to be a nurse-led service.
This means participants and their support networks can feel reassured that support is not only practical, but also observed through a clinical lens.
Health-related support is not always directly funded under a participant’s plan, but where it is appropriate and within scope, we nurture and support health and wellbeing through observation, referral, reporting, and advocacy.
Our trained support workers document what they see on shift. Those notes matter. They may show changes in mood, mobility, appetite, continence, medication routines, skin integrity, behaviour, sleep, pain, safety, or general wellbeing.
Having a Clinical Nurse involved means those concerns do not just sit in a file.
They can be reviewed, understood, escalated, and acted on appropriately.
Where needed, HiYa can:
* identify concerns through intake and ongoing support notes
* assess risk and changes in presentation
* refer to appropriate health professionals
* communicate with support coordinators, families, guardians, and allied health teams
