Regularly reviewing your goals and checking your progress can help ensure your supports continue to meet your needs as your circumstances change.

Here's how you can stay on track:

📋 Review your goals regularly and celebrate your achievements.
🤝 Stay connected with your Local Area Coordinator (LAC) or Support Coordinator for guidance.
📝 Update your plan if your needs or goals change over time.
💙 Ask for support—NDIA staff and NDIS Partners can help with applications, gathering evidence, and reviewing your plan.
📞 Need assistance? Contact the National Contact Centre on 1800 800 110 to be referred to your local NDIS partner.
