Operation: Abdominoplasty with muscle repair.

Outcome: Before and 6 months post-operative. This patient presented with post-pregnancy abdominal muscle separation and excess lower abdominal skin. Her abdominal muscles were repaired to improve abdominal wall support and help restore waistline contour. A total of 1.245 kg of excess lower abdominal skin and tissue was removed. The scar was positioned low, within the underwear line where possible.

Surgery duration: 2 hours.
Hospital admission: Inpatient stay for 3 nights. 
Recovery: Compression garment for 6 weeks.

Operation: Bilateral breast reduction.

Outcome: Immediate on-table result. For this patient, enlarged breasts were contributing to neck and back discomfort. A total of 998g of breast tissue was removed, 530g from the right and 468g from the left, to reduce breast volume while reshaping and lifting the breasts. Her breast asymmetry was also addressed.

Surgery duration: 2.5 hours
Admission: Overnight stay
Recovery: Compression garment for 6 weeks

Operation: Excision BCC (R) nasal alar rim + full thickness skin graft.

Outcome: Before and after 4 weeks following an excision of an aggressive BCC to her right nasal alar rim. This case involved a full thickness resection of her alar rim. This is a challenging area to reconstruct. A full thickness skin graft was used and rolled to reconstruct the rim. This is the result, there is no significant notching or collapse of the rim.

Surgery duration: 30 minutes
Hospital admission: Day Surgery

Great day yesterday speaking with medical students at a Careers and Skills Clinic hosted by my good friend and general surgeon Gian Capati.

It was a great reminder of why I chose plastic and reconstructive surgery. The diversity, creativity and precision involved across reconstructive and cosmetic surgery make it an incredibly rewarding specialty. I particularly enjoy giving students an insight into the breadth of what plastic surgeons actually do.

Outcome: Before and after 3 months post-operative. Significant breast enlargement had contributed to neck and back discomfort for this patient. Bilateral breast reduction surgery was performed, removing a total of 1640g of tissue (810g from the right and 830g from the left). The breasts were lifted and reshaped, with reduction of the areolae. Neck liposuction was performed concurrently.

Operation: Revision deep plane face and neck lift.

Outcome: Before and 6 weeks after surgery. This patient presented following a previous facelift combined with massive weight loss, resulting in recurrent laxity and abnormal scar tethering. Revision surgery involved careful release of scar tissue and a deep plane face and neck lift to improve jawline definition and address neck laxity at a structural level.

By working in the deep plane, the lift repositions the underlying facial structures rather than just tightening the skin, allowing for a more durable and natural result, which is particularly important in revision cases.

This patient also underwent a limited abdominoplasty (mini-abdominoplasty) at the same time.

Surgery duration: 5.5 hours.
Hospital admission: Overnight stay.
Recovery: Facial support band for 6 weeks.

Operation: Immediate breast reconstruction following bilateral mastectomy. A two-stage implant-based approach was performed using tissue expanders followed by definitive implants.

Outcome: Before and 1 year post-operative following mastectomy. This patient underwent bilateral mastectomy for triple negative breast cancer, with immediate reconstruction using 350cc tissue expanders placed in a submuscular plane with acellular dermal matrix. At the second stage, these were exchanged for 450cc Ergonomix full profile implants to restore breast shape and contour.

Operation: Removal and replacement of breast implants.

Outcome: Before and 2 months post-operative. This patient underwent implant revision surgery for ruptured 385cc implants, downsized to 340cc. Careful repositioning allowed improved symmetry, with a noticeable reduction in the gap between the breasts and an improved contour despite the downsizing.
