Hunter Primary Care is a not-for-profit organisation located in Newcastle NSW. For over 30 years we have been supporting people to live a healthy life by delivering a range of health and wellbeing services to the Newcastle and Hunter community. Our services include -
• Aboriginal & Torres Strait Islander Health Services including care coordination and chronic disease management, mental health car

e coordination and psychology services
• Aged Care services including Aged Care Emergency (ACE), Aged Care Nurse Practitioner, Aged Care Clinical Placements Program and psychology services
• Allied Health services including Priority Allied Health, Multidisciplinary Lifestyle Community Groups and Coordinated Respiratory Support
• GP Access After Hours
• Mental Health Services including headspace Newcastle, Commonwealth Psychosocial Support and psychology services
• NDIS Services including Group Programs, Level 3 Specialist Support Coordination, Positive Behaviour Support, Psychology and Allied Health including dietitians, occupational therapists, exercise physiologists and social workers. Connect.

🎂🎈 Recently, the Soul Clinic team celebrated one year of supporting guests at Soul Hub. 🎈🎂
A partnership between Hunter Primary Care and Soul Hub Newcastle, Soul Clinic provides accessible healthcare in a safe, welcoming and trusted environment for people experiencing homelessness, disadvantage and complex health challenges.
In its first 12 months, the clinic delivered:
💙 198 clinics
💙 1,893 GP appointments
💙 Support for 693 guests
💙 Care for 305 people accessing the service for the first time.
While we're proud of these milestones, we're even prouder of the relationships built, the lives impacted and the partnerships that make this model of care possible.

This week, Hunter Primary Care acknowledged National Reconciliation Week and National Sorry Day with a special staff event aligned to this year’s theme, ‘All In’ 🖤💛❤️
We were honoured to welcome Uncle Justin Ridgeway from the Worimi Local Aboriginal Land Council and Murrook Culture Centre, who shared powerful reflections on identity, relationships, spirituality, truth-telling and connection. Staff were encouraged to reflect on the role we all play in reconciliation and how we can continue to learn, listen and walk together.
