A patient with uncontrolled diabetes and PAD underwent a reverse sural artery flap placed over exposed tendons. Dr. Austin Mishko didn't have to guess whether the flap was viable; he used SnapshotNIR which gave him a real number, in real time: StO2 71% immediately post-op. The next day this was confirmed with follow up imaging, giving the clinical team confidence in their intervention.
The result was a wound prepped for a split thickness skin graft in the upcoming weeks, and ultimately, a limb that didn't have to be amputated.

A 49-year-old man was at high risk of amputation. Here's what changed the outcome.

Dr. Austin Mishko, DPM, and his team at Lee Health took a staged approach, eventually performing a reverse sural artery fasciocutaneous flap to cover the exposed tendons in the wound. The question remained; would the flap survive long enough for the wound to progress towards healing, and the next step of the treatment plan?

Dr. Mishko used SnapshotNIR to gather objective data in real-time. Immediately post-op, StO2 read 71%. On post-op day 1, still a strong 67%. The patient was able to progress to the next steps of treatment, ultimately healing and avoiding amputation.

Those are StO2 readings from the same wound, before and after a single conservative cleansing.

Here's what guided the clinical decisions:

SnapshotGLO revealed extensive red and cyan fluorescence throughout a fungating melanoma wound, bacterial burden beyond what visual assessment had suggested. The clinicians completed the intervention and SnapshotNIR confirmed the result.

Peri-wound and wound bed oxygenation both climbed, moving the tissue towards progressive healing, measured in the same visit, with no delayed cultures and no repeat appointment needed to know if it worked.

Bacteria and oxygenation are two of the biggest barriers to healing. This case shows what happens when you can see, and treat, both.
