Ending homelessness isn't just about providing housing, it's about creating the conditions for people to rebuild their lives.
Every person who transitions out of homelessness represents improved health, greater stability and a stronger connection to their community.
Our 2025-2026 Impact Report shares how community-based care is making that impact possible, one person at a time.

The future of healthcare isn't just about treating illness, it's about preventing crises before they happen.
Community-based care connects people with primary care, outreach and wraparound supports that improve health outcomes while helping reduce reliance on emergency departments.
It's one of the many ways NECHC is demonstrating that investing in care closer to home benefits everyone.

Some impacts are measured not just by numbers, but by responsibility.
NECHC manages 89.9% of Nova Scotia's permanent supported housing for high-acuity tenants, individuals with some of the most complex health and social needs.
This reflects more than the scale of our work. It reflects the trust placed in community-based care to support people with compassion, dignity and coordinated services that help them remain safely housed and connected to care.

What if the future of healthcare isn't a new idea, but one that's already delivering results?

NECHC's newly released 2025 - 2026 Impact Report provides compelling evidence that community-based care is improving lives and reducing pressure on the healthcare system.

As NECHC CEO Marie-France LeBlanc puts it:

"The question is no longer whether this Model of Care works. It's how quickly we can make it available to more Nova Scotians."
