The Trans Peer Outreach program offers supports and services for transgender and gender diverse people, to assist them to improve the quality of their lives through the provision of community-based, client centered care that supports positive interdependence. Services are delivered by people with lived experience as a transgender or gender-diverse person, and who also may have experience living wi
th a mental health concern. Trans Peer Outreach services are founded upon the principle that peers can better understand, relate to and support individuals with similar experiences, related to the health and wellbeing of people within this population, and are in a unique position to offer assistance. One to one Peer Support services can provide assistance in the following areas; reducing Isolation and/or self-isolating behaviour, developing wellness tools/strategies, assistance with accessing supportive communities, exploring gender identity, development of resources for navigating and accessing Health Care, and information and strategies to assist with transitioning. We seek and welcome collaboration with others who share our goal of creating, providing and improving services for trans, non-binary and gender diverse people throughout our communities.
