We’re looking for a compassionate, energetic Healthcare Assistant to support a young autistic man to live independently and enjoy an active lifestyle.

The role may involve long walks, gardening and a range of physical and social activities based around his interests.

Please note:

- A high level of physical fitness is essential, including the ability to walk, or run if needed, at least 8 km per day at a fast pace.
- Applicants must be non-smokers for health reasons.
- Being a driver would be advantageous.

🌱Training should never be something we simply complete and move on from.

In health and social care, training gives people the knowledge, skills and confidence to respond well when it matters most, whether that’s during an emergency, when someone is distressed, or in the everyday decisions that shape the quality of support.

For us, the real value of training is what happens afterwards.

Are people more confident?
Are teams working more consistently?
Are we making better decisions?
And most importantly, is the person receiving better support?

That’s the difference between attending training and putting learning into practice.
Because the outcome isn’t the certificate.

💚Moving from hospital into community-based care requires careful planning, consistency and the right support around the person.

Our Bridging Support is designed to support this transition, bringing multidisciplinary expertise into the process before, during and after the move.

⏱️When a placement is at risk of breakdown, understanding what’s happening is just as important as responding quickly.

Periods of crisis can be incredibly challenging for the person, their family and the professionals supporting them. But a placement reaching a difficult point doesn’t mean we should stop looking at what can be done differently.

We work closely with commissioners and wider multidisciplinary teams to understand the individual’s needs, identify risks and explore the factors contributing to the current situation.

Our focus isn’t simply on managing the immediate crisis. It’s about creating the right support around the person to help restore stability and work towards more sustainable, long-term outcomes.

⏱️When a situation is urgent, being able to respond quickly matters. For urgent and complex referrals, support can be mobilised in as little as two hours where circumstances allow.

💚Jack's journey is a wonderful example of what can happen when support is built around the person.

After spending 18 months moving between temporary homes, Jack now has the stability to focus on what matters most to him: enjoying life. Today, his days are filled with trips to the beach, swimming, time outdoors and moments shared with family.

One of the milestones we're especially proud of is seeing Jack return to swimming. Through careful planning, positive risk-taking and the right support, something that had once been out of reach became possible again.
His journey continues, with new goals, new opportunities and every reason to look forward to what's next.

We believe great support is about more than meeting needs. It's about helping people build confidence, embrace new opportunities and live lives that are meaningful to them.
