🔬 REVISION RHINOPLASTY | EXTRACORPOREAL SEPTOPLASTY WITH RIB CARTILAGE RECONSTRUCTION

Chris, aged 32, presented following previous nasal surgery performed elsewhere, which left him with significant functional and aesthetic concerns. He was experiencing severe breathing difficulties due to marked septal deviation, with the nose deviating significantly to the right. Adding further complexity to his case, Chris had previously undergone excision of a basal cell carcinoma from the dorsum of the nose, reconstructed using a full-thickness skin graft taken from the neck. This creates additional surgical challenges, as the skin and soft tissue envelope must be carefully elevated whilst preserving blood supply and graft integrity.

To correct the deformity, Consultant Facial Plastic Surgeon Dr Fas Arshad FRCS (ORL-HNS) performed an advanced extracorporeal septoplasty. This highly specialised procedure involves removing the entire septum from the nose, reconstructing it externally, and creating a new, straight structural framework.

A small 2.5cm incision was made on the chest to harvest rib cartilage, which was then meticulously sculpted to create a new septum and central support structure for the nose. This allows restoration of both nasal function and facial balance, improving airflow, structural stability, and aesthetic appearance.

The immediate on-table result can be seen towards the end of the video, demonstrating improved midline alignment, enhanced profile definition, and a straighter basal view.
