This is the scan of an elderly lady👵, right?... Wrong ❌
This is a REMS scan of a 35-year-old, 54kg, healthy, active female! 😳
Despite her apparent fitness, the bone mineral density (BMD) of her femoral neck was 0.699g/cm², giving her a T-Score of -1.4. This places her in the Osteopenia bracket for the area scanned.
📉 Breaking down the metrics:
T-Score: Compares your bone density to that of a healthy young adult. A score between -1.0 and -2.5 indicates Osteopenia (low bone mass).
Z-Score: Compares your bones to what is "expected" for someone of your same age, s*x, and ethnicity.
Fragility Score: This is the "So What?" metric. It looks at the actual quality and microarchitecture of the bone. Thankfully, her score was a safe 10.2, meaning her 5-year fracture risk remains low (4-8%)
🧪 The "Unintentional Recipe" for Osteopenia
How does this happen to someone so young?
- Living most of her busy, stressful life (Cortisol hinders bone-building cells and encourages more bone breakdown)
- Whilst borderline underweight (load on the skeletal system promotes bone strength)
- With her sporting history being low-impact sports (eg. swimming),
- Having a diet low in dairy and calcium (vital for strong bones),
- Often working until the early hours, impairing sleep quality (Growth Hormone and Melatonin are primarily secreted during deep sleep)
- And the classic UK, low sun exposure and resultant diminished natural Vitamin D production, created the perfect storm for bone loss.
This was an accidental finding with eye-opening implications 😲
