Fees for forms & administrative services

Please note that paperwork completed outside of a consultation, such as forms, medical certificates, reports and other administrative requests, may incur a fee.

Charges typically range from $20 to $52, depending on the complexity and time required. In some cases, it may be more appropriate and cost-effective to book an appointment with your GP or Nurse Practitioner to discuss and complete the paperwork together.

A full list of our fees is available on our website, at reception and in our waiting room. If you are unsure whether a fee applies, or would like an estimate of the cost, please ask our reception team when making your request.

💙 Bowel cancer is one of the most common cancers affecting men in New Zealand, but when it's found early, it can often be successfully treated.

Symptoms can include:
✔️ Blood in your bowel motions
✔️ Changes in bowel habits
✔️ Ongoing abdominal discomfort or bloating
✔️ Unexplained weight loss
✔️ Persistent tiredness

While some risk factors can't be changed, there are steps you can take to reduce your risk, including eating a healthy, high-fibre diet, exercising regularly, maintaining a healthy weight, limiting alcohol and avoiding smoking.
Bowel cancer often develops slowly and may not cause symptoms in its early stages. That's why screening is so important.

📬 Free bowel screening is available every two years for eligible people aged 60–74 years.
If you have symptoms, a family history of bowel cancer, or questions about your risk, please get in touch. We're here to help.

❤️ Heart disease is one of the leading causes of death for men in New Zealand, claiming more than 3,600 male lives each year.
The good news is that many of the risk factors can be reduced through healthy lifestyle choices.

You can help protect your heart by:
✔️ Being smokefree
✔️ Managing your blood pressure and cholesterol
✔️ Staying physically active
✔️ Maintaining a healthy weight
✔️ Limiting alcohol
✔️ Managing stress
✔️ Keeping blood glucose levels under control

Heart disease often develops gradually and may not cause obvious symptoms in its early stages, which is why regular heart health checks are so important.
If you're due for a heart check, or you're unsure of your risk, get in touch with our team. We're here to help.
