We honor the nearly 3,000 lives lost, the families who continue to carry their loved ones in their hearts, and the brave first responders, healthcare workers, military members, and everyday Americans who showed extraordinary courage in the face of unimaginable tragedy.

We remember the sacrifice.
We honor the heroes.
We cherish the unity that brought our nation together.

25 years later, we still remember.

🎉 Exciting News at Trinity Primary Care! 🎉

We're growing to better serve our community!

💙 Starting August 31st, Trinity Primary Care will be OPEN ON MONDAYS! This means even more opportunities to schedule appointments and receive the personalized care you deserve.

As part of these exciting changes, our Walk-In Clinic for acute illnesses and minor injuries will be available Monday through Friday from 1:00 PM to 4:45 PM, making it even easier to get the care you need when you need it.

We're also excited to welcome Dawn Taylor, MSN, APRN, FNP-C

Dawn Taylor’s passion for healthcare began over 22 years ago as a Nurse Tech while earning her Associate in Science of Nursing from Valencia College. She went on to complete her Bachelor in Science of Nursing at Jacksonville University, and in 2021, celebrated a special milestone alongside her sister, Denise, as they both earned their Master of Science in Nursing (Family Nurse Practitioner) from AdventHealth University. Dawn has been board-certified by the American Association of Nurse Practitioners (AANP) ever since.

Throughout her multi-faceted 22-year career with AdventHealth, Dawn served in diverse clinical and leadership roles—from frontline Nursing and Senior Nurse Manager to Clinical Nurse Analyst and Family Nurse Practitioner. Her dedication to compassionate, high-quality care has consistently earned national recognition, placing her in the 99th percentile for Press Ganey patient experience scores. Most recently, she had the privilege of caring for families locally at Grace Pediatrics and Wellness.

At the heart of Dawn’s practice is patient empowerment. Having navigated her own personal journey with obesity and weight related conditions, she combines authentic lived experience with clinical expertise to offer compassionate, judgment-free support. She works closely with each patient to build sustainable lifestyle changes, manage chronic conditions, and achieve long-term wellness.
