Gurwin offers skilled nursing care, short-term rehabilitation, medical/post-surgical sub-acute care, respiratory and ventilator care, social and medical model adult day care programs, hospice and palliative care, home care, Memory Care, acute care and dialysis. Gurwin's 36-acre campus is also home to a renowned assisted living community, Gurwin Jewish ~ Fay J. Lindner Residences, and Fountaingate Gardens, a new and unique independent living community for active older adults. The System also includes it's newest member, Island Nursing and Rehab Center located in Holbrook, Long Island.

Celebrating 37 Years of Care & Connection!

Gurwin’s Adult Day Health Program at Gurwin Jewish Nursing & Rehabilitation Center celebrated 37 years with a week full of fun, including a fashion show, carnival games, an awards ceremony and more! We’re proud to provide compassionate care, meaningful activities and social connections.

Our Adult Day Health Care Program at Gurwin Jewish Nursing & Rehabilitation Center had a wonderful time creating their own charcuterie boards! With a delicious variety of cheeses, fresh fruits and crackers to choose from, everyone got to customize their boards just the way they liked them. The best part? Sharing smiles, conversation and great company along the way!

It was a fun and tasty way to bring everyone together!

Our residents at Gurwin Jewish Nursing & Rehabilitation Center had a Dolly Parton-themed luncheon filled with some of Dolly’s favorite Southern comfort foods! From chicken and dumplings and warm biscuits to mac and cheese and fried green tomatoes, there was plenty to love. With delicious food, great company, and lots of laughs, it was a hootin’ good time for everyone!

Gurwin Jewish Nursing & Rehabilitation Center its annual Pumpkin Stroll, bringing plenty of creativity and fall fun! Our talented staff and departments transformed ordinary pumpkins into unique, imaginative creations that made their way through the halls of the Center. Staff and residents joined in the fun by voting for their favorites!

A big thank-you to everyone who participated and helped make this festive fall tradition so much fun! Can't wait to see who the winner is!

Our Adult Day Care Social Program had a wonderful time celebrating the season with a delicious apple-tasting experience and pumpkin picking and decorating on the green!

Everyone enjoyed sampling a variety of apples, including Honeycrisp, Ginger Gold, Red Delicious, Gala and Fuji. From tasting these fall favorites to choosing and decorating pumpkins, it was a day filled with seasonal fun, smiles and wonderful memories!

There’s nothing like enjoying the simple pleasures of fall together!

Our residents had a howling good time at Gurwin’s Wag-o-ween Dog Costume Parade!

Our adorable four-legged guests dressed to impress as they strutted down the runway at Gurwin Jewish Nursing & Rehabilitation Center, bringing plenty of smiles, laughter and tail wags along the way! A special shout-out to our board member, Pam E., who brought her precious pup, Tanner, dressed as a crocodile. His costume was a big hit with our residents!

Thank you to all our furry friends and their humans for making this such a fun and memorable event. We can’t wait to do it again!

Happy Fall, Y’all! Our Gurwin Adult Day Care Social Program at Gurwin Jewish Nursing & Rehabilitation Center had a rootin’-tootin’ good time celebrating fall with a fun cowboy-themed day filled with games, laughter and great company!

The festivities continued with a special luncheon featuring roasted Cornish hens, apple cranberry salad, mashed potatoes and green beans, along with refreshing mint tea and cold apple c***r.

Good food, fun activities and wonderful company made for a memorable day filled with fall fun and smiles!

Our residents at the Gurwin Jewish Nursing & Rehabilitation Center celebrated National Dessert Day with a delicious sampling of sweet treats! From creamy custard topped with fresh berries and whipped cream to bread pudding drizzled with vanilla sauce and a rich brownie paired with vanilla ice cream, there was something to satisfy every sweet tooth. Served with coffee or tea, it was the perfect way to indulge, share a little sweetness and enjoy time together!

Kindness in Action at Gurwin!

We are grateful to Commack High School student Hamza Daisman, who is volunteering with Embrace Relief and has chosen Gurwin as the recipient of a generous donation in support of our residents!

Through this thoughtful initiative, Hamza and Embrace Relief have secured winter slippers, hats and handbags to help bring comfort and joy to our residents.

We sincerely thank Hamza and everyone at Embrace Relief for thinking of our residents and for their generosity. Community-minded young volunteers like Hamza remind us of the meaningful difference that kindness and giving can make!
