At Legacy Medical, our goal is to provide comprehensive health care that is accessible, cost-effective and improves the health and quality of life for your residents. We provide patient-centered, in-home primary care services within residential senior living communities, including Assisted Living, Memory Care and Skilled Nursing Facilities as well as within the private homes of our area seniors. We understand it may be difficult for your residents to leave their home
for health services. The primary care provider will deliver ongoing care and will recommend specialty services as needed.

Please help us welcome Hailee Smith, MSN, APRN, FNP-BC, and Ivy Brosseau, DNP, APRN, FNP-BC, to the team!

Both bring a wealth of nursing experience and a passion for caring for older adults. Hailee spent nine years working in hospital and home health settings, while Ivy’s background includes critical care, nursing education, and clinical leadership.

Hailee will be seeing new patients in the Sioux Falls area, and Ivy will be seeing patients in Bismarck and Jamestown.

Recovery doesn’t end when rehab does.
