TMJ dysfunction isn’t just jaw pain; it often shows up in ways people don’t immediately connect.

Headaches, ear fullness, neck tension, facial pressure, and even teeth grinding can all be linked back to how your jaw is functioning.

Meet Jake.

PT aide, patient care coordinator, furniture assembler, exercise demonstrator, conversation facilitator, and the reason this clinic actually runs.

Jake has been with Koah since the very beginning. And we mean the very beginning β€” before there were patients, there was Jake putting together furniture. When I was too pregnant to get on the floor to demonstrate exercises, Jake was already on the ground doing it. Whatever the clinic needs, Jake figures it out.

Patients love him. Sometimes a little too much. Occasionally I have to physically break up the conversation so we can stay on schedule.

Off the clock he is rock climbing, gaming, or somehow flying to New York on a red eye for the weekend and back at the clinic Monday morning like nothing happened. Highly caffeinated at all times. Never seems to slow down.

Koah would not be what it is without him. We are really glad he is here.

Imaging can provide useful information, but it doesn’t always tell the full story.

Some people have significant MRI findings with very little pain. Others experience persistent symptoms despite minimal imaging changes.

Pain is influenced by many factors, including:
β€’ movement patterns
β€’ nervous system sensitivity
β€’ recovery capacity
β€’ stress and lifestyle factors
β€’ strength and mobility

Treatment should focus on the individual β€” not only the image.

Recovery is rarely a straight line.

Some weeks feel easier. Some involve flare-ups, fatigue, or slower progress. That doesn’t automatically mean healing has stopped.

Progress can also look like:
β€’ improved movement confidence
β€’ fewer symptom flare-ups
β€’ increased activity tolerance
β€’ recovering faster after discomfort

The body often needs time to adapt, especially after chronic pain, surgery, injury, or long periods of compensation.

Small improvements still matter.

Migraines are not random. They have patterns, and those patterns usually make sense once you know where to look.

For many people, recurring migraines are connected to things that have nothing to do with the head itself. Neck mobility restrictions. Jaw tension that's been there for years. A nervous system running at capacity before the migraine even starts. Breathing mechanics that load the upper cervical spine without anyone realizing it.

These aren't separate issues. They're often part of the same picture.

Physical therapy isn't the first place most people think to go for migraines. But for a lot of our patients, it's where things finally started to shift after years of managing symptoms without real resolution.

This is one of the most common things we hear. And it almost always means the same thing: the pattern driving the problem hasn't been found yet.

Most care is organized around symptoms. Where it hurts. What to avoid. What to stretch. That's not wrong, but it's often incomplete.

Beneath the symptom is usually a story. How the body is loading, compensating, regulating, and adapting. When you start working at that level, things shift in a way that actually holds.

If you've been stuck, you're not out of options. You may just need someone looking at a different layer.

If you treat the headache but not the jaw, or the neck but not the ribs, you're managing symptoms, not solving anything.

Jaw tension, neck pain, and headaches are rarely separate problems. They share muscles, nerves, and movement patterns. When one is dysregulated, the others feel it.

This is why so many people cycle through treatment without lasting relief. The right area isn't being addressed because no one is looking at how they connect.

At Koah, this is one of the most common patterns we see, and one of the areas where a whole-body approach makes the biggest difference.

If you've been told there's nothing more that can be done beyond medication, there often is.

Born in Colombia, endlessly passionate about movement, and yes, she'll probably beat you at soccer.

Stefany specializes in both orthopedic and pelvic floor physical therapy, and she brings the same whole-body thinking that Koah is built on. She understands that pelvic floor symptoms rarely exist in isolation β€” and that people deserve care that actually looks at the full picture.

Most people with hypermobility have been told they're "too flexible."

What they're rarely told is why they're exhausted.

When joints don't have enough stability, the muscles around them never get to switch off. They're working overtime, all day, just to hold things together. That constant effort accumulates, and it shows up as fatigue, tension, recurring headaches, and pain that never quite makes sense.

Stretching more doesn't fix this. Your body isn't asking for more mobility. It's asking for support.
