Most families don’t realize they’re in a crisis… until something finally forces a decision. 🤍

The challenge is that major moments—like a hospitalization or a fall—usually don’t happen out of nowhere. There are often smaller warning signs first:

• mobility changes
• medication confusion
• missed meals
• increasing forgetfulness
• difficulty managing daily routines

And because those changes happen gradually, families often normalize them for longer than they should.

💛 The earlier you start paying attention and exploring support, the more options you typically have—and the less pressure you feel later.

Planning early isn’t giving up independence. It’s protecting safety, dignity, and peace of mind for everyone involved.

If you’re starting to notice changes and feel unsure what the next step should be, Navacare can help guide you through your options with clarity and support before things become overwhelming.

One of the most overwhelming moments for families is leaving the hospital and suddenly being handed a list of next steps. 🤍

In this conversation, Sherri Franceschini from PrimeCare Home Health explains how home health services are arranged after a hospitalization or rehab stay—and what families should know before making a decision.

Many people don’t realize:

👉 You have the right to choose your home health agency.

During discharge planning, physicians, case managers, and social workers may recommend home health services based on your loved one’s needs. Families are then typically given agency options to review and select from.

That’s why it’s important to:

• research ratings and reviews
• understand the services offered
• ask questions about communication and care quality
• make sure the agency is the right fit for your loved one’s needs

💛 The goal isn’t just getting services started—it’s making sure your loved one receives the right support at home.

If you ever feel unsure navigating these decisions, Navacare can help guide you through your options with clarity and confidence.

This clip is from a recent Navacare-produced event, where we convened experienced professionals across the senior care continuum to provide meaningful, real-world insight for families navigating complex decisions.

Many families don’t think to ask about visitation policies until after their loved one has already moved into assisted living. 🤍

But this is actually a really important question to ask early in the process.

A community should support connection with family—not create unnecessary barriers around it.

💛 While every community may have reasonable guidelines to protect residents’ comfort and safety, overly restrictive visitation policies can sometimes be a red flag worth paying attention to.

When touring communities, consider asking:

• What are the visitation hours?
• Are there any limitations for family visits?
• Can family participate in meals or activities?
• How does the community communicate with families?

The goal is to find a place where your loved one feels supported—and where families still feel involved, informed, and welcome.

Choosing a community isn’t just about the apartment or amenities. It’s about the overall environment, communication, and quality of life.

At Navacare, we help families know what questions to ask so they can make more confident care decisions.

Caregiving doesn’t just exhaust the body.
It exhausts the heart, too.

The constant awareness.
The decision-making.
The quiet worry running in the background of everyday life.

That kind of tired goes deep.

This week’s 1-minute caregiver reset is a reminder that you do not have to push through every moment. Even strong caregivers need rest. 🤍

Pause. Breathe. Reset gently.

Falls rarely happen “out of nowhere.” 🤍

Often, there are smaller warning signs first:

• walking slower
• holding onto furniture
• struggling with stairs
• losing balance more often
• avoiding activities they used to do easily

Because these changes happen gradually, they’re easy to dismiss as “normal aging.”

But paying attention early can help prevent bigger problems later.
