🧠 Generalized Onset With Focal Evolution (GOFE) Seizures: A Rare and Newly Recognized Seizure Pattern in Generalized Epilepsy
A 33-year-old with a history of generalized epilepsy demonstrated classic 3–4 Hz generalized spike/polyspike-wave discharges on EEG. While some seizures remained generalized, others began with a generalized pattern and then evolved into focal activity in the left temporal region, accompanied by automatisms and head version. One event progressed to a convulsion. MRI was unremarkable.
💡This pattern is consistent with Generalized Onset with Focal Evolution (GOFE) - a rare but recognized phenomenon in generalized epilepsy where seizures start generalized but later localize.
