Pain is not the only symptom of endometriosis and this single fact is responsible for so much missed and delayed diagnosis.

Between 20 and 25% of women with endometriosis have no pain symptoms at all, which means a full quarter of cases are quietly progressing while everyone involved assumes no pain means no problem.

Around 40% of women experiencing infertility will be found to have endometriosis, and a significant number of them find out this way, not through period pain, but through a fertility workup after months or years of being told everything looks normal. If youve had unexplained infertility, there is up to a 50% statistical chance that undiagnosed endometriosis is a factor, which is a number that should be part of every fertility conversation and somehow rarely is.

What actually shows up instead of pain can be heavy or irregular bleeding, chronic fatigue, bloating, or nothing noticeable at all until conception becomes the problem.

This is why preconception care matters.

Normal is the range that rules out disease. Optimal is the range that builds a pregnancy.

Those are two different ranges and nobody tells you that, so when your results come back with no flags you assume you got an answer to the question you were asking. You didnโ€™t. You got an accurate answer to a different question.

A reference range is a population range. Itโ€™s built around whatโ€™s common, and common includes a lot of women in their 30s walking around with iron stores that are technically fine and wouldnโ€™t survive nine months and a birth.

The clearest example is ferritin. A low-normal ferritin isnโ€™t anaemia and it isnโ€™t a store either. Itโ€™s a balance you can run down. Now put a pregnancy on top of it.

Same number on the same page. Two completely different verdicts, depending on which question you brought to it.

Things your man should know about your cycle.

Women can only get pregnant 2-3 days a month. Her egg survives just 12-24 hours after ovulation. But menโ€™s s***m can survive up to 5 days, which is why our actual conception window stretches to around 5-7 days.

Womenโ€™s hormones shift every single week of our cycle. We wonโ€™t feel the same Monday as we did last Monday. Our energy, appetite, need for connection, all of it changes. We need different things at different times, and thatโ€™s okay.

In the luteal phase, itโ€™s completely normal and healthy for women to eat more carbs. Womenโ€™s body is running warmer and working harder. We also need more Vitamin C during this time to support progesterone production, which is essential for a healthy conception environment.

Women genuinely need more sleep than men, especially in the follicular phase. This isnโ€™t laziness. Female sleep needs are different, and protecting rest is protecting our chances.

Understanding her cycle isnโ€™t just sweet, itโ€™s one of the most practical things you can do to support her journey to conceive.

Being fertile AF is not just about having babies. I know that sounds backwards because the word fertility gets used like its only relevant once youre standing in the pregnancy test aisle deciding if this is the year, but ovulating regularly every month is actually your body running a full systems check on your hormones, your thyroid, your stress load and your metabolic health, all at once, whether you ever want kids or not.

A cycle that shows up on time every month means your brain and your ovaries and your adrenals are all talking to each other properly, and a cycle that goes missing or gets weird for months at a time is usually your body waving a flag about something else going on, not just a reproductive issue sitting off in its own little corner.

And this is not some minor detail either, ovulation is the main way your body makes progesterone, which is doing quiet work on your sleep and your mood, and estrogen from regular ovulation is also protecting your bone density long before you will ever notice it matters, which is why athletes and women who lose their periods from overtraining or undereating can end up with bone loss in their twenties that does not always come back once their cycle does.

So if you have been treating your period as background noise until the day you decide to try for a baby, I would gently suggest flipping that, because by the time youre trying is a strange time to start paying attention to a system that has been talking to you the whole time.

Alcohol changes how your body handles oestrogen. It ramps up an enzyme called aromatase, which converts testosterone into oestrogen. More aromatase activity means more oestrogen being made in the first place.

And I havenโ€™t even gone into what this is doing to your gut and liver on top of that. Two systems you need working for you, not against you, if youโ€™re trying to get your hormones sorted.

I used to say have a drink every now and then, itโ€™s fine. I donโ€™t say that anymore. If youโ€™re serious about your hormones, just give it up. Find another social outlet. This one is genuinely bad for your hormones, and no amount of moderation talk changes that.

If you live with endometriosis, adenomyosis, PMS, or youโ€™re trying to conceive, oestrogen dominance doesnโ€™t create these conditions out of nothing, but it feeds whatโ€™s already there.
