Recovering from a health setback? The RCP might be the program for you to help you get back on your feet and stay independent at home🩵

The Restorative Care Pathway is a short-term program, up to 16 weeks, designed to help you recover after an illness, injury, or hospital stay.

A multidisciplinary team, including physiotherapists, occupational therapists, nurses, and dietitians, will work together under a dedicated restorative care partner to support your recovery goals, with funding available to help you along the way.

PlanCare is proud to offer the Restorative Care Pathway program to help you restore daily living skills, manage health changes, and build long-term independence.

Under the Australian Government's Support at Home program, funding is delivered as a quarterly budget and once you understand how it works, it is actually quite simple! It arrives in four equal payments across the year, one for each quarter, so you always know what is coming.

Before you spend it, up to 10% is set aside for your Care Management, covering all the planning and support happening behind the scenes for you.

The rest? That is yours to enjoy on the care and services that matter most to you.
And here is the best part: if you do not use it all, you are not penalised. Your unspent funds simply roll over to the next quarter. There is just one cap to keep in mind, you can carry forward up to $1,000 or 10% of your quarterly budget, whichever is greater.

With PlanCare, you are always on track. To help you keep an eye on your budget, we send automated email notifications as your quarterly funds are used, so you will hear from us each time you reach a milestone within the quarter. No surprises, just clarity. 🩵

Here is the comparison: traditional domestic support costs around $120/hr. With self-management through PlanCare, it is closer to $71/hr, including our 9.8% Platform Management Fee.

CAPS stands for the Continence Aids Payment Scheme and it is a separate annual government payment (not part of your Support at Home budget) that helps older Australians cover the cost of continence products like pads and other aids.

CAPS was removed for Support at Home and CHSP clients when the new Aged Care Act began in November 2025. But after strong feedback from clients, carers and health professionals, the Government has reversed that decision.

From 1 July 2026, CAPS has been restored. The current payment rate is $739.40 per year and it does not come out of your regular Support at Home funding.

If you previously had access to CAPS and lost it, you will need to reapply. But don't worry, your Co-Care Partner can help you work out what to do next 🙋🏻‍♀️🩵

From 1 July 2026, Support at Home funding amounts have been updated through annual government indexation. For many clients, that means more support available each quarter🩵

Funding now ranges from $11,000 up to $80,000 per year, depending on your classification.

Budgets are released quarterly, and clinical care services such as nursing, physiotherapy and occupational therapy remain fully government-funded on top of your regular budget✅

If you are thinking of getting some extra support with your newly updated funding, now is a great time to check in with your co-care partner to update your care plan.
