Advance Care Planning Canada

Advance Care Planning Canada ? Who would you trust to make your healthcare decisions? April 16th is the Canadian National Advance Care Planning Day.

It is a day for you and others to reflect on decisions you would like made about your health care, should you be unable to speak for yourself. Advance Care Planning Day provides an opportunity to open up a conversation with a loved one about their wishes for end of life care - or it might remind you to review your own plan.

Many people think advance care planning is only for older adults. It's not.

Unexpected illness or injury can happen at any age. Advance Care Planning helps ensure the people who care about you understand your wishes and preferences if you're ever unable to speak for yourself.

Whether you're 18 or 108, having these conversations matters. What would you want someone to know about what matters most to you?

June is National Indigenous History Month. As we reflect on the profound histories and enduring strength of First Nations, Inuit, and Métis Peoples, CHPCA and ACP Canada are reaffirming our commitment to reconciliation in healthcare.

We believe high-quality hospice and palliative care must be rooted in cultural safety, humility, and deep respect for diverse traditions. Reconciliation goes beyond this month of reflection. It must be an ongoing, daily practice of listening, learning, and collaborating to dismantle barriers to compassionate end-of-life care.

Join us in honouring . Together, let’s work toward an equitable healthcare system for all. . . . .
