Stonegate Community Health Centre Stonegate CHC has been providing primary health care, health promotion and a wide variety of community.

Together, we identify individual and community needs and respond by providing accessible and inclusive health services and health promotion programs. We specialize in helping those living with chronic conditions and those whose health is at risk because of poverty. We offer more than 35 different medical, counselling and community programs. If we cannot fill a need, we partner with other agencies

to coordinate care for our clients. We know that true health can never be achieved unless our clients have access to an adequate income, safe housing, nutritious food and fell truly connected to their community.
