🌟 What an incredible journey in Ljubljana!

The in-person sessions of the ERN ReCONNET Transition Training Course have officially concluded, but our shared commitment to improving care for young people living with rare and complex connective tissue diseases (rCTDs) is stronger than ever.

Over the past few days, healthcare professionals, paediatric and adult specialists, and patient representatives came together in Ljubljana to address one of the most critical phases in healthcare: the transition from paediatric to adult care.

Together, we explored:
🔄 Transition models, readiness, planning, and care continuity.
🗣️ Effective communication strategies with young people and their families.
💻 Digital tools and interactive workshops to translate theory into daily clinical practice.

A heartfelt thank you to our outstanding faculty, dedicated participants, and speakers for the extraordinary energy, passion, and active engagement throughout the course!

📚 The journey doesn’t end here!
Participants will now continue with the 6-week virtual learning programme, featuring interactive case discussions, peer exchange, and dedicated mentoring. Together, the in-person and virtual components make up the full course, EACCME®-accredited for up to 21.0 European CME credits (ECMEC®s).

Let’s keep working together to build seamless, continuous, and patient-centred care pathways across Europe! 💙

🌟 Day 2 of the ERN ReCONNET Transition Training Course is underway!

Today in Ljubljana, the focus moves from theory to practice: Transition Care in Practice.

Today’s sessions explore:
🔄 Transition models, readiness and planning
💬 Communication with young people and families, including sensitive topics
📱 Digital tools and interactive workshops to put skills into practice.

Because a successful transition is not just about transferring care: it is about listening, planning together, and building trust with young people and their families. 🤝

A big thank you to our faculty and to all participants for the energy and engagement shown so far. Let’s keep going! 💙

We are pleased to announce the upcoming ERN ReCONNET webinar:
“The management of reproductive health and pregnancy in Antiphospholipid Syndrome (APS): answers to the patients’ questions”

Planning a pregnancy with antiphospholipid syndrome? What are the real risks, and how can they be managed safely?
Join our webinar to explore the questions patients ask most often about obstetric APS, from preconception counselling and pregnancy management to fertility, contraception and reproductive health. We'll look at hypertensive disorders of pregnancy, such as preeclampsia and HELLP syndrome, and what they mean for both mother and baby. We'll also cover long-term cardiovascular and thrombotic outcomes for women, the interplay between APS and lupus, and how children born to mothers with APS do over time. Throughout, we'll discuss anti-thrombotic treatment during and outside pregnancy in line with current clinical guidelines, with particular attention to bleeding risks.
