๐Ÿšจ Clinical Pearl #10: Lamotrigine โ€” Start Low, Go Slow

One of the most important safety concerns with lamotrigine (Lamictal) is serious rash, including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). Risk increases with rapid titration and with valproate, which increases lamotrigine concentrations.

๐Ÿ’Š Standard adult bipolar titration without valproate or enzyme inducers:
Weeks 1โ€“2 โ†’ 25 mg daily
Weeks 3โ€“4 โ†’ 50 mg daily
Week 5 โ†’ 100 mg daily
Week 6 โ†’ 200 mg daily

Lamotrigine + new rash = take it seriously.

Assess medication interactions, titrate appropriately, and educate patients to report a new rash promptly. Valproate requires a lower starting dose and slower titration.
