You woke up and your shoulder wouldn't lift past your ear. No injury, no fall, normal X-ray β€” and a vague suggestion that this is just what your forties feel like.

It may not be. For a lot of women, it's what falling estradiol feels like.

There's a name for it now: the musculoskeletal syndrome of menopause. More than 70% of women experience musculoskeletal symptoms across the transition, and about a quarter are genuinely limited by them β€” joint pain, stiffness, muscle loss, accelerated bone loss, frozen shoulder.

Estrogen isn't only a reproductive hormone. It regulates inflammation, supports collagen in tendons and joint capsules, protects muscle, and protects bone.

One caveat worth naming: frozen shoulder is also strongly associated with diabetes and thyroid disease. Which is exactly why it deserves a real workup.

And please don't let 65 be the default for your first bone density scan β€” that's when it's routinely covered, not a biological deadline.

One in three midlife women could book it. Almost nobody does β€” because it feels like a confession instead of what it actually is: a symptom with a workup.

If a clinic only offers pellets, ask why.

Once a testosterone pellet is implanted, it cannot be adjusted and it cannot be removed. Pellets frequently push levels well above the normal female range β€” and if side effects show up, there's no way to turn the dose down. You wait it out.

Hormone therapy should be able to respond to how you actually feel. So it's a fair question to ask out loud: "If this dose doesn't suit me, how do we adjust it?" Listen carefully to the answer.

You brought it up at your appointment. Desire that used to show up on its own doesn't show up at all β€” and your drive, at work and at the gym and at home, feels like it's running on a dimmer switch.

And you heard one of two things: "your labs are normal," or "we don't really do that here."

Here's what you deserve to know: testosterone isn't a men's hormone that women borrow. Your ovaries and adrenal glands make it, and for much of adult life you have more circulating testosterone than estradiol. Levels decline gradually through the 40s and beyond β€” and treating that decline, in the right patient, is real medicine.

So why is it so hard to find a doctor who will prescribe it? The honest answer is a training gap: there's no FDA-approved testosterone product for women in the US, so prescribing it well means off-label dosing, careful titration, and follow-up labs. Most physicians were never taught it β€” so they either don't offer it, or offer one delivery method to everyone.

Started estradiol and watched the scale creep up a few pounds? Take a breath β€” that number is measuring mass, not fat.

During the menopause transition, low estrogen is associated with tissue changes throughout the body β€” skin, muscle, joints, and the genitourinary tract can all become drier and less resilient. When estradiol is restored, some of those tissues may rehydrate and rebuild. Muscles may hold more water and stored fuel. Skin and connective tissue may plump back up. That recovery has weight.

Meanwhile, research links estradiol to a healthier pattern of fat distribution β€” less of the visceral fat that tends to accumulate around the middle after menopause. So the scale can rise a little while body composition is actually improving.

Two honest caveats: estradiol can cause temporary water retention, breast tenderness, or bloating β€” especially around dose changes β€” and those usually settle. And a lower weight isn't automatically a healthier one; frailty weighs less than strength.

The better question isn't "am I gaining weight?" It's "what kind of tissue am I gaining?" That's exactly what a body composition assessment and a real hormone conversation can answer.
