Bluegrass PACE Care Outreach Coordinators Gerri Stuart and Mark Johnson have recently had some wonderful opportunities to connect with our community and share how Bluegrass PACE Care and Bluegrass Care Navigators support individuals and families across Kentucky.

Recently, our Bluegrass PACE Care team hosted a PACE Yourself Lunch & Learn at Bethsaida Baptist Church, where we shared information about Advance Care Planning, Home Modification, Hospice, and PACE Care. We were grateful for the engaged attendees, thoughtful questions, and meaningful conversations that helped strengthen community partnerships.

We also had the honor of participating as a vendor at the Commonwealth Center for Fathers & Families Summit. The event provided a great opportunity to share information about PACE, connect with community partners, strengthen relationships, and gain valuable insights that help us better serve our communities.

Thank you to everyone who joined us at these events. We appreciate every opportunity to connect, educate, and build partnerships that support healthier, more independent lives. 💙

A big thank you to Hazard Provider Liaison Ashley Farler for representing Bluegrass Care Navigators at ARH’s Hot Cakes for Hope Breast Cancer Awareness event and Mountain After Hours Clinic’s Trunk or Treat! There are only a few days left to register for our 2026 Children’s Grief Awareness Day Event!

In recognition of Children’s Grief Awareness Day, families are invited to stop by our open house to honor loved ones who have died through meaningful, family-friendly activities. Guests can create a craft, paint a remembrance rock, add it to our garden in honor of someone special, or take it home as a keepsake. Supportive resources and conversation will also be available for grieving children and families.
