One of the biggest barriers to healthcare AI transformation may not be the technology. It may be the way we organize ourselves.

AI is bringing that challenge into sharper focus.

An AI initiative may involve IT, compliance, privacy, clinical operations, revenue cycle, finance, and the people who actually perform the work.

When each group operates independently, critical knowledge can get lost, risks can fall between departments, and implementation can become more difficult than it needs to be.

I believe healthcare organizations need to start organizing AI initiatives around outcomes, not just departments.

That means bringing the right expertise together, connecting knowledge across appropriate boundaries, and involving the people who understand the work in how AI is designed and implemented.

But breaking down silos does not mean weakening accountability. As teams become more connected and AI becomes more involved in workflows, clear decision authority, human oversight, and governance become even more important.

The goal isn’t to eliminate specialization. It’s to prevent specialization from becoming isolation.

The future of healthcare AI isn’t just about building smarter technology. It’s about building smarter organizations around it.

I explore this in my latest blog, “Breaking Down the Silos: Why AI Transformation Requires a More Connected Healthcare Organization.”

When healthcare organizations evaluate AI, one of the first questions is often, “How accurate is it?”

That matters, but after more than 40 years in healthcare compliance, I believe we need to look beyond the algorithm.

AI risk can come from the data a system accesses, the vendor behind it, the people using it, the decisions it influences, and the controls (or lack of controls) surrounding it.

Even a highly accurate model can create significant risk if the organization has not established clear ownership, appropriate access, meaningful human oversight, and ongoing monitoring.

That is why I believe AI governance must address the entire environment, not just the technology.

I continue to come back to a few essential questions:
Who owns the system?
Who is accountable for the decisions it influences?
Can people verify its sources, challenge its recommendations, and stop the workflow when necessary?
As AI becomes more capable, those questions become more important, not less.

My perspective is simple: the risk isn’t just the algorithm. It’s the ecosystem we build around it.

Not every AI risk should be managed the same way.

As I continue studying AI leadership and drawing on more than 40 years in healthcare compliance, I believe we need to be more precise about the risks we are trying to manage.

An AI system can fail operationally, create strategic challenges even when it works as designed, or introduce unexpected risks we could not have anticipated before deployment.

That is why I use a simple framework: Control. Position. Watch.

Control the operational risks we can identify. Position the organization to address strategic risks involving vendor dependency, workforce impact, and regulatory change.

Watch for unexpected behaviors and emerging risks that require us to reassess our assumptions and respond.

We will not identify every AI risk before deployment. What matters is whether we have the monitoring, human oversight, and authority to intervene when circumstances change.

As I often say, build the brakes before you need them.

Throughout my career in healthcare compliance, I’ve learned that establishing expectations is only the first step. We also need to verify that the right controls are in place and working.

We develop policies, train people, implement controls, and then audit and monitor to make sure they are working. When something goes wrong, we investigate the root cause, implement corrective action, and verify that the problem has been corrected.

So why would we govern AI any differently?

AI can produce a confident answer that is wrong, miss an important exception, or behave differently as models, data, and workflows change. Testing before deployment is essential, but it is only the beginning.

I believe healthcare needs to apply the discipline of traditional compliance to AI governance: continuous monitoring, meaningful human oversight, root-cause analysis, corrective action, and validation.

And when experienced professionals repeatedly override an AI recommendation, we should ask why. Those disagreements may reveal risks the system does not recognize.

AI governance cannot stop at deployment. As I often say, don’t wait for perfection but don’t scale chaos.

After more than 40 years in healthcare compliance, I know how much valuable knowledge organizations build over time.

It lives in regulatory interpretations, audit findings, corrective action plans, payer requirements, policies, and (just as importantly) in the experience of the people who understand how the work actually gets done.

But I also know how easily that knowledge becomes fragmented, outdated, or difficult to find when it is needed most.

As healthcare organizations adopt AI, I believe we need to pay closer attention to the knowledge behind the technology. AI can retrieve information and connect ideas, but we still need to know whether the source is authoritative, whether the information is current, and whether it is appropriate to use.

My perspective is simple: before you build your AI strategy, build your knowledge strategy.

AI does not replace the expertise an organization has spent decades developing. The opportunity is to make that expertise more accessible, connected, and useful without losing the governance and human judgment that make it trustworthy.

We talk a lot about who needs to be in the room when healthcare organizations deploy AI.

Executive leadership. IT. Compliance. Legal. Privacy. Security. Operations.
But after more than 40 years in healthcare compliance, I believe we also need to ask a different question:

Who is missing from the room?

The people with the most organizational authority are not always the people with the most operational knowledge. And the people with the least organizational power may be the ones who experience the greatest consequences when AI gets something wrong.

That is why I believe AI stakeholder mapping in healthcare needs to look beyond the organizational chart.

We need to listen to the people who understand the exceptions, see where the workflow breaks down, recognize when an AI recommendation does not make sense, and may be affected by the outcome without having any authority over the technology.

And we need to create an environment where they feel empowered to say:
“This doesn’t look right.”

That is not resistance to AI.
It is risk identification.

AI adoption is accelerating in healthcare. But responsible AI is not created by principles alone.

After more than 40 years in healthcare compliance, I have learned that having the right policy language is only the beginning. The real work is translating principles like fairness, transparency, accountability, privacy, security, and equity into controls that can actually be implemented, monitored, audited, and enforced.

With AI, that means asking harder questions:

What controls are in place?

Who owns the decision?

Can we trace the source?

How are we monitoring performance?

What happens when AI gets it wrong?

And who has the authority to intervene?

That is why I believe responsible AI needs to move from good intentions to operational governance.

AI may participate in a decision, but AI cannot own accountability for that decision.

And as I often say, “Show me the source.” Because fluency is not evidence.

Responsible AI is not simply about having principles. It is about being able to demonstrate that those principles are built into the way AI is governed.

Read more about my perspective in my latest blog post, “Responsible AI in Healthcare: Principles Aren’t Enough, You Need Controls.”
