Acupuncture Before and After Embryo Transfer
Supporting Uterine Blood Flow and Reducing Ectopic Pregnancy Risk.
For patients going through in vitro fertilization, embryo transfer day carries enormous emotional weight. Everything up to that point — the medications, the monitoring, the retrieval — has been building toward this single procedure. It's natural to want every possible advantage going into it. One of the most well-studied complementary approaches used around embryo transfer is acupuncture, and research suggests it may help in two distinct ways: by supporting blood flow to the uterine lining beforehand, and by calming uterine muscle activity afterward.
Before Transfer: Supporting Blood Flow to the Uterine Lining.
The lining of the uterus, known as the endometrium, needs a rich blood supply to support an embryo after it implants. A thin, poorly perfused lining is one of the more common reasons an otherwise healthy embryo fails to implant.
Acupuncture treatment given in the days surrounding embryo transfer has been studied specifically for its effect on this blood supply.
Researchers commonly use a technique called Doppler ultrasound to measure how easily blood moves through the small vessels feeding the endometrium. A randomized, single-blind controlled trial published in the Chinese Journal of Integrative Medicine used three-dimensional power Doppler ultrasound to compare women who received acupuncture treatment against a group who received a sham (placebo) version, tracking blood flow across the days surrounding egg retrieval and transfer.
Across multiple randomized and observational studies, acupuncture in the pre-transfer window has been associated with reduced resistance in the vessels feeding the endometrium, increased endometrial thickness, and better markers of local blood supply.
A separate randomized controlled trial in patients with a thin endometrium, published in Frontiers in Medicine, found that combining acupuncture with standard hormone therapy produced a significantly greater increase in endometrial thickness and volume, along with better blood flow scores on ultrasound, compared to hormone therapy alone. In that trial, the group receiving acupuncture also had a meaningfully higher clinical pregnancy rate — 55.3 percent compared with 31.1 percent in the control group — and a higher embryo implantation rate overall.
A broader review of this research area, published in 2025, concluded that acupuncture administered before and after embryo transfer is associated with reduced blood flow resistance in the vessels feeding the endometrium, greater endometrial thickness, and physiological markers consistent with a better-nourished, more receptive uterine lining — the combination reproductive specialists refer to collectively as endometrial receptivity.
After Transfer: Calming Uterine Muscle Activity
The second window of interest is immediately after the embryo is placed in the uterus. This is a moment that gets far less public attention than the transfer itself, but it may matter just as much for where the embryo ultimately settles.
Passing the transfer catheter through the cervix and into the uterine cavity can trigger a brief, reflexive contraction of the uterine muscle — sometimes described as uterine spasming. Because the junction where the uterus meets the fallopian tube sits so close to where the embryo is deposited, even a short contraction at that junction can, in theory, redirect a freshly placed embryo away from the uterine cavity and into a tube, where it has no ability to develop safely.
This is the leading explanation offered in the literature for why ectopic pregnancy is more common after IVF than after natural conception.
The numbers illustrate the difference clearly. Ectopic pregnancy occurs in roughly 1 to 2 percent of pregnancies conceived naturally. After IVF embryo transfer, published estimates put the rate meaningfully higher — commonly cited as approximately 2 to 9 percent of clinical pregnancies, or roughly two-and-a-half to five times the background rate. Uterine contractility at the moment of transfer is considered one important piece of that larger picture.
This is where post-transfer acupuncture has been studied as a complementary approach.
Acupuncture is understood to engage the parasympathetic nervous system — the body's "rest and digest" branch — and to lower circulating stress hormones such as cortisol. Both effects are associated with reduced smooth-muscle activity in the uterus. By calming this reflexive contraction in the minutes and hours after transfer, acupuncture treatment may reduce the likelihood that an embryo is displaced into a fallopian tube, supporting a normal, uterine implantation instead.
Clinical outcome data adds further support. A large retrospective cohort study presented at the American Society for Reproductive Medicine looked at more than 2,300 frozen embryo transfer cycles at a single fertility center.
Patients who received individualized acupuncture treatment for 30 minutes before and immediately after transfer had a 55 percent higher live birth rate and a 42 percent lower rate of biochemical (very early, non-viable) pregnancy compared with patients who underwent transfer alone, with the study also tracking ectopic and miscarriage rates as secondary outcomes.
Putting the Two Effects Together.
The value of acupuncture around embryo transfer isn't really one single effect — it's the combination of two effects, each addressing a different physiological need at a different moment:
● Before transfer: improved blood flow builds a thicker, better-nourished endometrium, creating a more receptive environment for the embryo to attach.
● After transfer: a calmer, less reactive uterine muscle reduces the chance that the embryo is displaced away from the uterine cavity, lowering the relative risk of ectopic implantation.
Together, these two windows — one focused on preparing the tissue, the other on protecting the moment of placement — represent the physiological rationale behind treating embryo transfer as a same-day, two-part acupuncture visit rather than a single session.
A Note on the Evidence
What the evidence consistently supports is biological plausibility — measurable, reproducible changes in uterine blood flow and uterine muscle activity — paired with a number of clinical studies showing favorable associations with implantation and live birth rates.
