Peninsula Healthcare Connection Serving vulnerable populations by providing medical, case-management, and community resources. PHC services the north peninsula of the San Francisco Bay area.

Peninsula HealthCare Connection (PHC) is a not-for-profit organization with a focused mission to serve the needs of the homeless and the at-risk of becoming homeless. Medical, psychiatric and intensive case management is provided within a multi-service building complex with three other entities; Downtown Streets Team, InnVision, and the Housing Authority. The organizations operate cooperatively in

partnership to deliver health care, social services and housing. PHC provides services through a collage of community agencies consisting of staff, trainees and volunteers including the Palo Alto Medical Foundation and Stanford University as well as the generosity of many. Our common thread is our mutual commitment to the promotion and maintenance of human dignity, health and safety.

We’re excited to kick off a new chapter in building a stronger culture of belonging at PHC!

With support from Destination: Home, we’ve launched a multi-year effort to center lived experience and belonging across our organization.

Last week, we held our first Cultivating Belonging workshop with Regina Lawless Toney, an energizing start to this work. Over the next two years, she’ll partner with our staff and Board through trainings, dialogue, and initiatives that help ensure everyone can show up fully as themselves.

anxiety, and trauma in ways similar to us even if they are more at risk of being overlooked by
healthcare systems - they work, raise children, and pursue personal goals all while receiving
unique supports through primary and specialty care focused on alleviating burdens of cost,
predictability, and consistency.

PHC's Permanent Supportive Housing program celebrates housing 45 individuals and families at Ira D. Hall Square, a 100% affordable housing community in Sunnyvale. Through partnership with MidPen Housing and Santa Clara County's Office of Supportive Housing, PHC will provide supportive services to residents as they integrate into the larger community to define what a new home means under a building named for a MidPen Housing Co-Founder committed to ending housing discrimination.
