We know transitions like this can feel overwhelming, and we’re here to help make things as easy as possible! 😊

A few important reminders:
📅 Any appointments previously scheduled with GTIFM did NOT automatically transfer to West Front Primary Care.
➡️ You will need to contact WFPC to schedule appointments with our team.
📞 Call (231) 935-8930 and select option #3

📋 Medical records from GTIFM also did NOT automatically transfer to WFPC. We recently learned that some paper Medical Release forms completed before GTIFM closed may not have been processed.

The good news? 🌟
Your WFPC provider WILL still see you and help guide your care, even if records have not arrived yet. During your first visit, your provider will discuss what records may be helpful moving forward.

We know changing medical offices can feel a little confusing sometimes, but we’re here to make the transition as smooth and supportive as possible! 😊 As an independent practice, some of our policies and procedures may look a little different than what you’re used to.

Here are a few helpful things to know as you get settled in with our WFPC family:

🌟 SAME DAY CARE
We work hard to offer same day appointments for our patients, including evening hours and Saturday availability for urgent needs!
🕗 Monday–Thursday: 8am–8pm
🕗 Saturday: 8am–1pm

💊 MEDICATION REFILLS
Please request refills BEFORE you run out! Refill requests may take up to 2 business days to complete.
📞 231-935-8930 ext. 1

⏰ ARRIVING FOR APPOINTMENTS
Please plan to arrive 15 minutes early so we can keep our busy schedule running smoothly for all patients. Late arrivals may need to be rescheduled.

📅 NO SHOW / LATE CANCELLATIONS
We kindly ask for at least 24 hours notice if you need to cancel or reschedule your appointment. WFPC’s No Show/Late Cancellation (less than 24 hours) fee is $50 for first time, $100 for second time, and $100 plus possible discharge for third.

Whenever possible, we’ll schedule you with your own WFPC provider. If they’re unavailable, one of our Same Day Care providers will step in to help care for you. 😊

✔️ You MUST complete New Patient Forms before we can fully document you in our system
✔️ GTI records did NOT automatically transfer

We strongly encourage you to complete your onboarding NOW instead of waiting until an urgent medical need or emergency pops up. ⏰

The good news? We DO have procedures in place to help with urgent needs and medication refills while patients get established with our team. 🙌
