Staying active as you get older can help you maintain your independence, improve your balance, strengthen your muscles and bones, reduce your risk of falls, and boost your mental wellbeing. ๐Ÿšถโ€โ™€๏ธ

The NHS recommends that older adults should:
โ€ข Aim to be physically active every day.
โ€ข Do activities that improve strength at least twice a week.
โ€ข Include exercises that improve balance and flexibility, especially if you're at risk of falling.

It is not just our clinical staff whose professional development we support!

Meet Emily ๐Ÿ‘‹

Emily joined Team Castle as a Care Coordinator and recently completed training with us to become a Social Prescriber Link Worker. She had the following to say:

โ€œIโ€™ve recently completed my Social Prescribing training, to become a Social Prescriber Link Worker. My training has allowed me to gain more of an understanding on how to support patients through a holistic, person-centred approach to health and wellbeing.

My role is to look beyond medical needs and understand the wider factors that can impact a personโ€™s wellbeing, including social, emotional, practical and community needs. I enjoy working alongside patients to identify what matters most to them and connect them with appropriate services, resources, activities and community support.

I am so grateful for the support from colleagues and the management team for the support to complete this training as I feel better equipped to help patients to make positive changes, build confidence, strengthen connections and improve their overall quality of life.โ€

Meet Beth ๐Ÿ‘‹

Beth is one of our wonderful Practice Nurses. Beth is currently part way through completing her Advanced Professional Practice Masters degree. She recently completed her independent prescribing course. She is also currently training to be able to consult with patients regarding long acting reversible contraception including IUDs (coils) and IUSs (implants).

A super big well done to Beth for all her hard work and commitment she is putting into her continued professional development, she is a much valued member of the team at Castle Partnership. Beth had the following to say about her achievements:

โ€œAs part of my training on the Advanced Professional Practice Programme, gaining my prescribing qualification has been a valuable addition to my skill set. Iโ€™m incredibly grateful for the support from my manager and colleagues, for the time to study, and for the opportunity to learn from such knowledgeable clinicians at Castle. Iโ€™m proud to work alongside such fantastic colleagues who are always so supportive.

Bowel cancer is one of the most common types of cancer in the UK, but when it's found early, it's much easier to treat. That's why the NHS offers bowel cancer screening to people who are eligible. ๐Ÿ’ฉ

The screening test is simple, quick and can be done in the privacy of your own home. It looks for tiny amounts of blood in your poo that you may not be able to see yourself. ๐Ÿ 

If you receive your bowel cancer screening kit through the post, don't put it to one side โ€“ complete it and return it as soon as you can. Even if you feel fit and healthy and have no symptoms, taking part in screening can help detect problems early. โœ…

If you notice symptoms such as blood in your poo, a persistent change in your bowel habits, unexplained weight loss or ongoing tummy pain, don't wait for your screening invitationโ€”contact your GP. ๐Ÿ“ž

DNAโ€™s - โ€œDid not attendโ€ appointmentsโ€ฆ

Lots of discussion in the media recently around how many appointments are wasted in General Practice due to patients not attending.

We noticed this was a significant problem at Castle Partnership a few years ago, and did some digging to try and find the reason and the solution. Some are unavoidable, we are all human, and we can all simply forget (despite numerous reminders) or things happen outside of our control. But what we did discover is a huge proportion of these, appeared to be โ€˜repeat offendersโ€™ and therefore something we felt could be tackled.

Different practices take different approaches to this, comments from the public surrounding removing patients or charging patients for wasted time. We decided to see what we could do proactively firstโ€ฆ

We used our clinical system tools to identify patients who it predicted would likely not attend, we flagged them. Following this, on booking, a pop up would appear, notifying the person booking that the person is likely to DNA. Theyโ€™re then added to a ledger for our Care Coordinator Team.

The team proactively contact these patients ahead of their appointment, engaging with them, encouraging them to attend, or rebook, and with some - forming a relationship.

What we found is that these patients might have complex, chaotic lives, they might have a drug or alcohol dependency, they might be a carer, they might be in the midst of a domestic abuse situation, or be struggling with their mental health.

When you reach out and engage, this cohort of patients appreciate it - and they react, they attend, or they rebook! Not every time.. and thereโ€™s still the cohorts we canโ€™t tackleโ€ฆ but the data speaks for itself.

We have more appointments available now, and less DNAs. Not a massive figure, but approximately 780 appointments across a year.

Still more work to do, but we are very proud of what we have achieved as a team, and the icing on the cake is that we got those patients to their appointment, that they needed.

Next up on celebrating our team members recent achievements:

Meet Dan, Dan is a paramedic practitioner at Castle Partnership. Dan works in our home visiting team and also sees patients at the practice in clinic. He also works for the East of England Ambulance Service. He recently completed his independent prescribing qualification with us and had the following to say about it:

โ€œIโ€™ve just completed my Independent Prescribing qualification at UEA with support from Castle Partnership.

The course is renowned for being challenging, especially in its workload, so I was apprehensive about taking it on initially. However, I really enjoy building my knowledge to better support the patients I see, which is what drove me to push myself to complete this.

The learning from the course was fantastic and being able to contextualise this new knowledge to real patients, with the support and watchful eyes of the experienced staff at Castle Partnership was invaluable. The course not only developed my prescribing knowledge and clinical decision-making but also helped me reflect on and strengthen my existing practice.

Being granted protected learning time really made a difference for me โ€“ whilst I have strengths in learning and implementing new knowledge, I am also dyslexic, and academic writing does not come naturally to me. Having dedicated time was instrumental in helping me succeed.

I was so pleased to achieve a distinction after all the hard work from both myself and the staff at Castle Partnership who supported me throughout the course and who will continue to support me as I begin my prescribing journey. I am looking forward to realising the benefits of my new knowledge and skills to continually improve the care I provide to patients! THANK YOU!โ€

Continuing our theme of sharing our team members achievements, meet Rachael!

Rachael is one of our Practice Nurses at Castle Partnership and recently completed her independent prescribing course with us.

Rachael sees patients with long term conditions and chronic diseases, specialising in Diabetes management. Her continued professional development enables her to help patients further with managing their conditions.

We hugely value Rachaelโ€™s contributions to the Practice and the Nursing Team, she had the following to say about her recent achievement:

โ€œIn May this year, I successfully completed my Non-Medical Prescribing course. It was an intense but incredibly rewarding qualification, and Iโ€™m so pleased that I pushed myself outside of my comfort zone and took on the challenge.

Throughout the course, I had amazing support from my colleagues. I always felt comfortable asking questions, no matter how big or small. I felt fully supported throughout my learning journey and was grateful to be given dedicated study time to help me balance the demands of the course alongside my role.

Most importantly, it allows me to make patient-centred decisions and support patients more effectively as part of their overall treatment and care. Completing this qualification has been a real achievement for me, and Iโ€™m proud of the knowledge, skills and confidence Iโ€™ve gained. It has been a challenging journey, but one that has ultimately strengthened my practice and enabled me to provide an even better service to the patients I care for.โ€

Castle Partnership are committed to supporting our team to continue their professional development and career journeys. We are going to be sharing with you some of our team members recent achievements over the coming days, starting with our wonderful Paramedic Practitioner - Adam.

Adam has been working with us since 2024 within the Home Visiting Team, Adam also works for the East of England Ambulance Service. Adam recently completed his non-medical prescribing qualification ๐Ÿ™๐Ÿผ

Adam is a hugely valued member of the team and we look forward to continuing to support his development in any areas of interest in the future. He had the following comments regarding the course he completed and support he received:

โ€œI started my prescribing journey just over 18 months ago, with some understandable hesitancy and nerves prior to beginning a notoriously tough learning module. From the time I started prescribing, I was given additional time within clinics to find my feet and prescribe safely, with incredible non judgemental help whenever I needed it from my GP colleagues. The learning journey was difficult, but now I have the qualification, it makes my day easier, relieves prescribing duties off my colleagues and speeds up the time for which patients can receive urgent prescriptions.โ€
