At Humber River Health, we’re proud to see our teams driving meaningful change for Long-Term Care residents.

Beatrise Edelstein, our VP of Post Acute Care and Health System Partnerships, and Kathleen Kirk, Clinical Program Manager of our Schulich Family Medicine Teaching Unit and Integrated Care Services, recently presented at , highlighting the impact of our LTC++ Diagnostic Imaging & Transportation pilot in Northwest Toronto.

Through their integrated LTC–Acute Care Remote Monitoring Program, they created a streamlined pathway for LTC teams to coordinate directly with our LTC+ nurse navigator—making it easier to book diagnostic imaging and access fully funded, non-urgent transportation.

The results speak for themselves:
✅ 1,180 Emergency Department visits avoided
✅ $478K in cost savings across FY23/24 and FY24/25
✅ Improved care experiences for residents and families
✅ Greater confidence among providers in timely access to services

By removing transportation barriers, this initiative also advanced health equity for residents with limited mobility, social supports, or financial means, delivering care aligned with the .

We congratulate Beatrise and Kathleen on this wonderful initiative!

Have you heard about SCOPE (Seamless Care Optimizing the Patient Experience)?

SCOPE is a virtual, interprofessional care team that supports primary care providers through a single point of access—helping ensure patients receive the right care, at the right time, in the right place. Here’s one powerful example of how SCOPE is transforming patient care:

When primary care providers need urgent support, SCOPE helps connect the dots.

A patient with complex diabetic wounds required immediate guidance. Through SCOPE, their physician was quickly connected to a specialist, redirected to the most appropriate care pathway, and secured a timely vascular consult with Own Health’s Vascular Surgeons Drs. Asem Saleh and Kerry Graybiel.

The result: faster treatment, the right care team, and no unnecessary delays.

This is what coordinated, patient-centered care looks like.

Toronto is getting ready to welcome the world for the FIFA World Cup 2026TM, and Ontario’s health system is ready to support residents and visitors throughout the tournament.

Health system partners across Toronto and the GTA have been preparing for FWC26 to support access to care, emergency preparedness and system readiness.

Patients and visitors may experience increased traffic, road closures or delays near our three sites from June 11 to July 19. If you are coming to Hennick Humber Hospital, our Finch Campus, or our Church Campus for an appointment or to visit a patient, please allow extra travel time and check local traffic or transit updates before leaving.

Our community can continue to access care as usual through their primary care team, walk-in clinics, or calling Ontario Health 811 for help finding nearby care. If you require urgent or emergency care, please do not delay seeking care. If you are experiencing a medical emergency, call 911 or go to the closest Emergency Department.

Enjoy the matches, celebrate safely, and know where to turn if you need care!

This past weekend, Humber River Health and the Women’s Health Coalition of Ontario welcomed women from across our community for "Healthy Women, Stronger Communities: A Day Focused on Women's Health and Wellness." June marks the beginning of Pride Season, a term that refers to the wide range of Pride events that take place over the summer (June to September) when Two-Spirit, Le***an, Gay, Bisexual, Transgender, Q***r, and gender-diverse (2SLGBTQ+) communities and allies come together to spotlight the resilience, celebrate the talent, and recognize the contributions of 2SLGBTQ+ communities. Although special attention is put on the Pride events during the summer months, they happen throughout the year in many communities.

To honour this month, we have raised the Pride Flag at each of our three sites. We invite you to join us in celebrating Pride Season as we continue to build an inclusive culture grounded in respect, equity, and belonging for all members of our Humber River Health community.

👏 Celebrating our surgical teams!

Humber River Health has once again been recognized for outstanding surgical outcomes by the American College of Surgeons National Surgical Quality Improvement Program.

Only 76 of 609 hospitals received this recognition, and Humber is one of just three hospitals in Ontario to achieve top standing in both the All Cases and High Risk categories for over two consecutive years.

Thank you to our incredible teams for delivering exceptional patient care every day!

In honour of , meet Nabil, a physiotherapist supporting our patients at Hennick Humber Hospital.

Nabil’s interest in physiotherapy began as a young athlete recovering from injuries, where the care and attention he received inspired him to provide the same experience for his own patients.

In his role within the surgical units, Nabil assesses patients following emergency and elective orthopedic procedures, working closely with them to determine the level of support needed for recovery and to help set them up for the best possible outcomes.

For Nabil, improving patient outcomes starts with connection. By building strong relationships, understanding each patient’s goals, and providing individualized, culturally sensitive care, he and his colleagues help enhance both recovery and overall patient experience.

Recognizing Physiotherapy Month is an opportunity to acknowledge the contributions of professionals across the field, from clinical care to research, and to celebrate the collaboration and dedication that continue to strengthen healthcare.

This , meet Keerthi Jarajapu!

Keerthi’s journey into speech-language pathology was inspired by personal experiences. After watching her grandfather struggle with dysphagia, she saw how Speech-Language Pathologists helped make swallowing safer while still respecting the foods and cultures he loved. Volunteering with post-stroke patients also showed her how much communication can affect a person’s quality of life.

Today, Keerthi works at the Reactivation Care Centres, supporting patients in the post-acute phase of recovery after strokes, dementia-related changes, or medical illnesses. Her days are busy and fast-paced, involving swallowing and communication assessments, treatment, family support, and close collaboration with the healthcare team.

For Keerthi, one of the most rewarding parts of the role is helping patients safely return to the foods, drinks, and conversations that bring them comfort, connection, and joy.

Through her work, Keerthi hopes every patient feels heard, supported, and empowered during a difficult time in their lives. Her compassion and dedication reflect the important role Speech-Language Pathologists play in patient care every day.

Congratulations to our six graduating residents of the Schulich Family Medicine Teaching Unit.

Helping to support more than 5,000 patients in an area with one of the highest needs for primary care and among the lowest percentages of people with a family doctor in Ontario, these grads are a leading example of the safe, high-quality and equitable care we strive to provide at Hennick Humber Hospital.

Thank you also to the Schulich Family Foundation and The Slaight Family Foundation, without who's support, this program would not be possible.

Meet Helia, a Biomedical Engineer dedicated to supporting patient care through healthcare technology.

Helia’s day-today includes maintenance, troubleshooting, inspections, and responding to service requests for medical equipment. She plays a vital behind-the-scenes-role, helping healthcare teams deliver reliable and efficient care.

What drew Helia to Biomedical Engineering was the chance to combine healthcare, technology, and hands on problem solving in a meaningful way. One innovation that excites her most is the growing integration of smart technology and artificial intelligence into healthcare systems.

For Helia, the most rewarding part of the job is knowing her work directly supports patient care by ensuring safety, accuracy and efficiency for both patients and staff.
