✨ Three months after open piezo septorhinoplasty.

The biggest change here is the tip: better definition, improved tensioning and stronger support, creating a more refined shape that still looks entirely her own. 👃

A smoother profile and clearer dorsal aesthetic lines complement the tip, with carefully controlled rotation to preserve a natural, unoperated look.

🌬️ Function mattered just as much. Internal nasal valve augmentation was performed to improve the airway alongside the external shape.

😴 A particularly lovely piece of feedback: she reports better sleep, with her wearable recording more REM and deep sleep since surgery. These are personal device observations, rather than a formal sleep assessment, but a welcome part of her recovery.

Still early at three months, with residual swelling and further refinement to come.

Can AI really predict your final rhinoplasty result? 🔎

Following his recent feature in HELLO! Magazine, Mr Unadkat discusses some of the pitfalls of using AI when considering rhinoplasty.

More patients are arriving at consultations with AI generated or edited images showing the nose they hope to achieve. While these can be useful in helping a surgeon understand a patient’s goals, they cannot account for all the individual factors that influence what is realistically achievable through surgery.

So, how useful is AI when planning rhinoplasty, and where should expectations end?

Following on from our recent feature in HELLO! magazine, Mr Samit Unadkat has written for this month’s issue of Aesthetic Medicine 📖

In “AI in the Consultation Room”, he looks at a change he’s seeing in clinic. More patients now arrive with AI-“perfected” versions of their own face, or with algorithmic beauty reports, rather than a photo of someone else’s nose.

“When a patient brings in a photograph of someone else, there is an understanding that we are looking at a different person. When they bring in an image of their own face, the psychological distance collapses.”
He covers what AI can and can’t tell us, the rise of “beautymaxxing”, and why the best result is still a nose that looks natural, works properly and suits your face.

My advice to any patient who has used one of these tools: bring the report if you like. Then put it to one side, and let’s talk about your concerns, not the algorithm’s version of them.

What happens when rhinoplasty involves both appearance and breathing? 🔎

After considering rhinoplasty for some time and exploring options overseas, this patient came to Mr Unadkat with concerns about the shape of her nose as well as difficulties with her breathing.

Her assessment identified a prominent dorsal hump alongside a saddle nose deformity, where there was inadequate structural support within the nose. With both function and appearance to consider, the decision was made to proceed with a septorhinoplasty.

Using the patient’s before, post operative and on table photographs, Mr Unadkat talks through the case in detail and explains the surgical approach taken to help address her concerns and work towards her overall goals.

✨ Three months after a complex open structural rhinoplasty ✨

Truly honoured that this lovely patient put her trust in me 🙏🏽

Although the transformation looks natural and effortless, achieving it required careful structural planning beneath relatively tight, contracted skin.

Using the piezotome, I reshaped the bony framework and refined the dorsal profile. A septal extension graft was used to provide stable, controlled tip projection and support, while allowing the tip to be refined and positioned more centrally. The previous drift of the nasal tip towards the patient’s right has been corrected, together with the underlying septal deviation.

The radix was gently augmented to create a smoother transition from the forehead into the nose. This is a subtle but important detail: rather than simply reducing the nose, selective augmentation helps the new profile blend naturally with the rest of the face and preserves that all-important “unoperated” look that my patients desire.

From the side, the dorsal hump has been replaced by a smooth, elegant profile with better projection, rotation and facial balance. From the front, there is already a clear improvement in the dorsal aesthetic lines, nasal symmetry and tip shape, although considerable swelling remains at only three months and refinement will continue over the coming year.

Natural, structural and stable. A beautiful early result, with plenty more definition still to come. 👃✨

Rhinoplasty should always be personal to the patient 👃🏻

Rather than working towards one particular look, the focus is on the patient’s own anatomy, facial proportions and what they would personally like to address. The aim is a considered result that feels balanced with the features already there.

✨ Just two months after rhinoplasty, an elegant transformation for one of the loveliest patients I have had the privilege of treating.

Using an open approach and Piezo technology, I reduced the dorsal hump, significantly deprojected the nose with millimetric precision and softened the nasofrontal angle, while gently elevating and refining the nasal tip.

Even at this early stage, the profile is beautifully balanced, the dorsal aesthetic lines are clearly defined and the tip definition is already beginning to emerge; one of the great advantages of precise Piezo surgery. The result is softer, more elegant and entirely natural. 👃🏼✨

She had considered rhinoplasty for many years and feels that it has changed her life. I feel incredibly fortunate that my work allows me to meet such wonderful people.
